中文摘要 | 股骨近端的應力遮蔽效應是全人工關節置換手術失敗的原因之一,本研究以有 限元素法分析, 採用填滿骨髓腔內最大空間標準( mximum-canal-fill criterion )的植 入物設計,模擬不同披覆程度的多孔性披覆植入物植入股骨後,假設披覆部份與股骨有良好 接合的情形下,探討完全無披覆、植入物近端三分之一披覆、植入物近端三分之二披覆以及 完全披覆等四種不同程度披覆的植入物設計與應力分布的關係,並與完整股骨比較,期能找 出減少應力遮蔽效應發生的植入物設計,結果顯示,在股骨近端附近,披覆越少的植入物應 加遮蔽效應越小,完全披覆的植入物則在植入物末端有最小的應力集中現象。 |
英文摘要 | Stress-shielding effect contnues to be one of the major factors that cause failure in cementless total hip replacement. Therefore, the goal of this study is to use finite element method to investigate the stress-shielding effects of different porous-caoting extents on a maximum-canal-fill custom prosthesis. Four different extents of porous coating (un-coated, 1/3 coated, 2/3 coated, fully-coated) with the assumption of fully bone ingrowth to the porous-coated areas were considered. The stress distributions for the four implanted models and the intact femur model were com pare and the stree-shlding effects were evaluated Results shared that for the less coated prosthesis, less stress-shielding effect in the proximal femur. For the fully coated prosthesis, less stress concentration in the distal region near the tip of the prosthesis was found. |
