中文摘要 | 本研究目的在探討護理人員對執行身體約束的知識、態度和行為意向及病人約束照護之現況。採橫斷式調查法,利用立意取樣於台北某醫學中心之內外科加護病房護理人員,以三份研究工具:1.護理人員基本資料表,2.護理人員身體約束知識、態度、主觀規範和行為意向量表,3.約束護理品質查核表進行調查與評核。問卷共發出89份,有效問卷為59份,回收率為66.3%。資料以SPSS 16.0統計軟體進行描述性統計。結果呈現護理人員約束知識答對率為81.57%,約束態度及行為意向得分呈現傾向不對病人執行身體約束;在主觀規範結果顯示;決定執行身體約束時,受單位護理同仁、醫師、護理長影響。病人約束照護品質平均總分為8.46(1.50)分,但是對於約束前先執行其他替代方案部分,仍有加強努力的空間和必要性,回歸分析發現約束態度與主觀規範無法預測護理人員約束之行為意向,約束知識、態度、主觀規範及行為意向無法有效預測約束之品質。故本研究建議替代方案之在職教育強化護理人員對約束的態度,以及藉由同儕間彼此的影響以降低重症單位身體約束之使用。 |
英文摘要 | This study aimed to examine the factors related of critical care nurses knowledge and intentions (i.e. attitudes and subjective norm) toward the use of restraints on critical ill and the restrained patient care quality. A quasi-experimental designed study was utilized, and the study was conducted at a medical- surgical intensive care unit of a medical center; the questionnaires were sent t0 89 nurses, and 59 responded (response rate 62.7%). Three instruments were used: (1) nurses' demographics and professional characteristics, (2) a questionnaire of nurses' knowledge and intentions toward the use of restraints, and (3) a checklist of patient restraint quality. Data obtained were analyzed via SPSS 16.0 statistical package. The study results revealed that the average score of nurses' knowledge was 81.57%; thus, nurses had moderately favorable intentions and attitudes toward the use of restraints with critically ill patients, and subjective norms were the major factors (colleagues, doctors, head nurses) that influenced nurses' decisions regarding physical restraints. The average quality care score regarding the used of physical restraints was in a good range at 8.64 (1.50); however, the evidence showed that the alternatives of restraints were not an option for nurses before they made a decision to restrain agitated patients. Regression analysis found that attitudes and subjective norms were not significantly associated the intention to restrain, knowledge, attitudes, subjective norms and intentions were not significantly associated with the quality care of restraint. Further educational interventions focused on alternatives to restraints will be needed to reinforce the nurses' and their associates' attitudes on restraint and to reduce physical restraint rates in ICU. |
