中文摘要 | 目的:氣管內管是進行動物實驗前重要之麻醉步驟,對於最常用的小 型實驗動物大白鼠,其插管方法常需藉助特殊工具與技巧才能達成,而這些工具 並非一般實驗室所具備。本論文的目的是利用一個新設計的撐開器來輔助大白鼠 之氣管插管。材料與方法:我們設計了一個由3 mL塑膠針筒,塑膠針筒外環可 將大白鼠上下顎固定且撐開,針筒裁成一個斜面可將舌頭壓住,因塑膠針筒外環 直徑接近大白鼠口咽部打開之張開度,其斜面之設計已經抵住舌根,將此口咽部 撐開器,便可輕易地撐開大白鼠的口腔及咽喉,再藉由光源由前頸部照射便可清 楚觀察到會咽軟骨及聲帶並確定氣管入口,再將其管子迅速且正確放入氣管內 管,從開始至完成也僅需約30-45秒。結果:我們在不同實驗過程中共使用了42 隻,體重約350-500 g大白鼠,合併使用呼吸器及麻醉機上附加生命徵兆監測器 觀測後,可成功地麻醉且順利進行手術及實驗。討論:實驗動物的全身麻醉,若 以氣管切開術(tracheostomy)方式,則很難做長時間之觀察且易導致感染或其他併 發症出現,而影響實驗之長期進行。故成功的氣管插管是一個重要的實驗步驟。 以我們設計的撐開器可快速完成大白鼠的插管之步驟。而且在整個氣管插管過程 使用本撐開器既不影響也不傷及到大白鼠之氣管內管及口咽部。相較於以往報告 的方法,本法不需任何特殊工具也無技術上困難駕馭之缺點,很適合於做大白鼠 實驗時應用。 |
英文摘要 | Objective: A newly designed oral expander for endotracheal intubation in the rat was developed to provide a simple, economical, and reliable operative technique with readily available equipment. Material and methods: This method for intubation of the rat's trachea relies on the use of an oral intubation wedge, fashioned from a plastic 3-ml syringe, as an oropharyngeal expander. The oral wedge can be fixed firmly in the mouth by natural pressure from the palate and incisors. At the same time it displaces the tongue as well as provides appropriate expansion of the oropharyngeal cavity. Light from an ordinary table lamp directed on the anterior (ventral) aspect of the pharyngoepiglottic region, allows clear, direct visualization of the epiglottis and vocal cords through the wedge. Placement of the endotracheal tube becomes very easy, atraumatic, and rapid: it can be done within 30 - 45 seconds. Result: This oral wedge can be used even by those who lack experience because it provides direct visualization of the entrance of the trachea. Furthermore, because the oral wedge keeps the airway open smoothly during the whole course of intubation, it can be used for training beginners without unnecessary trauma to the animal. Conclusion: We have used the oral wedge many times during our electrophysiology studies and found it to be low-cost, simple, safe, and effective. |
