中文摘要 | 背景 臺灣中部地區常可見擴大型心肌病變患者,微量元素在原因不明性擴大型 心肌病變之致病機轉中,可能扮演一角色。至今尚無有關臺灣人之報告。 探討此種可能性, 作此一研究。 方法 吾人以32位擴大型心肌病變患者(24位男性、8位女性,平均年齡44��14歲)及 31位健康人(16位男性、l5位女性,平均年齡38��12歲)為對象,測定其血清硒、鋅、 銅、鐵濃度及血中谷胱�騛L氧化�蒗〝吽A並比較兩群間之差異。血清硒濃度以石墨電熱式原 子吸收光法來測定。血清鋅、銅及鐵濃度以火燄式原子吸收光法來測定。血中谷胱甘�騛L氧 化�蒗〝坅h以RANSRL法測定。 結果 發現擴大型心肌病變群之血清晒值為27.6��8.41μg/1比健康群之值47.2��14.7 μg/l低,其差異有統計學意義。血中谷胱甘�騛L氧化�蒗〝尪@度在擴大型心肌病變群亦比 健康群為低。在所有研究對象中,發現血清硒值及血中谷胱甘�騛L氧化�蒗〝坁膠釧�顯的相 關性。血清銅及鐵濃度在擴大型心肌病變群比健康群為高。血清鋅濃度在兩群之間未發現有 意義之差異。 結論 臺灣中部地區原因不明性擴大型心肌病變患者,其血清硒值及血中谷胱甘�騛L氧化�� 活性偏低,血清銅及鐵值偏高。 |
英文摘要 | Background. Sporadic cases of dilated cardiomyopathy (DCM) are often seen in central Taiwan. Though trace elements may play a role in the pathogenesis of idiopathic DCM, there are no reports concerning study of Taiwan's population in relation to trace elements in patients with DCM. Methods. Serum selenium, zinc, copper and iron concentrations and blood glutathione peroxidase (CPX) activity were measured in 32 patients with DCM 124 males, 8 females; mean age, 44��14 years) and 31 age-matched normal volunteers (16 males, 15 females; mean age, 38��12 years). Results. The average serum selenium concentration of 27.6��8.4 μg/l found in the DCM group was significantly lower than the average concentration of 47.2��14.7 μg/1 in the normal group. Blood GPX activity was also significantly lower in the DCM group than in the normal group. Serum selenium concentration correlated well with blood GPX activity in the studied subjects. Serum copper and iron concentrations were significantly higher in the DCM group than in the normal group. There was no difference in serum zinc concentration between the two groups. Conclusions. Deficiency of serum selenium in association with a low blood CPX activity, and high serum copper and iron concentrations, are found in patients with idiopathic DCM in central Taiwan. |
