中文摘要 | 背景:過敏因素在鼻息肉患者所扮演的角色,其重要性仍有爭議, 國內少有這方 面正式之報告, 因此我們試圖探討本地民眾鼻息肉與過敏之關係。 方法:從民國 82 年 7 月 1 日至 84 年 6 月 30 日止, 共有 68 名鼻竇炎併鼻息肉患者 被列入本研究中,探討其過敏體質,如氣喘、異位性皮膚炎、過敏性鼻炎、過敏家族中,藥 物過敏及食物過敏等病史之有無; 並分析皮膚試驗及血清中特異性 IgE 抗體之檢查結果。 且與國內外鼻息肉患者及正常人做比較。同時也探討具過敏因素者,其鼻竇炎併鼻息肉之病 灶是否會較嚴重。 結果:本研究中,鼻息肉患者併有氣喘病患者為 11.76 %( 8/68 ); 併有異位性皮膚炎 者為 13.3 %( 9/68 );併有過敏性鼻炎占 51.47 %( 31/68 );併有皮膚試驗呈陽性 者占 45.51 %( 31/68 ); 併有食物過敏者 11.76 %( 8/68 ); 併有藥物過敏者是 7.4 %( 5/68 );併有異位性家族史佔 25 %( 17/68 );併有血清特異性 IgE 抗體呈 陽性者占 42.6 %( 29/68 )。 依 Independent T-test 來比較過敏組與非過敏組其鼻竇 炎併鼻者肉病灶期數的平均值。 過敏組( 38 人)為 2.1,非過敏組( 30 人)為 2.0,p 值為 0.615,無統計上之意義。 結論:本研究中鼻息肉患者併有異位性皮鼻炎、過敏性鼻炎或皮膚試驗呈陽性的發生率與國 外患者之數據相似,但與一般人為統計對象之結果不同;至於鼻息肉患者併有藥物過敏或食 物過敏之發生率與國外鼻息肉患者及一般人相似。而有過敏因素者比無過敏因素者,並不會 使鼻竇炎併鼻息肉的病灶範圍變大。 |
英文摘要 | The ongoing debate of the role of allergic factors in nasal polyps continues. Still there are few studies reported in domestic literatures. In an attempt to determine the association between allergy and nasal polyps, we investigated 68 patients with nasal polyps, from July 1993 to June 1994, including 43 men and 25 women with age from 14 to 74. The clinical data included asthma, atopic dermatitis, allergic rhinitis, atopic family history, drug allergy and food allergy. Skin tests with 12 allergens and 22 specific IgE screens were performed. We also explored whether these allergic factors would exacerbate the associated sinusitis in these nasal polyp patients. The incidences of asthma, atopic dermatitis, allergic rhinitis, positive skin test, food allergy, drug allergy, atopic family history, and positive serum specific IgE antibody in our nasal polyps patients were 11.76% (8/68), 13.3% (9/68), 51.47% (31/68), 45.51% (31/68), 11.76% (8/68), 7.4% (5/68), 25% (17/68), 42.6% (29/68), respectively. According to Independent T-test, the mean value of sinusitis with nasal polyps in allergic group (38 people) and non-allergic group (30 people) were 2.1 and 2.0 respectively. P values 0.615 and there was no statistic meaning. We conclude that the incidences of atopic dermatitis, allergic rhinitis, and positive skin test in these patients were similar to those of nasal polyps patients in overseas but different from those of the general population. Nevertheless, the incidence population. Nevertheless, the incidence of food allergy or drug allergy in nasal polyp patients was similar to those of the general population. Furthermore, there was no statistic difference between the allergic group and the non-allergic group with regards to the stage of the associated sinusitis. |
