中文摘要 | 骨質疏鬆症被認為是胰島素依賴型糖尿病合併症之一。但有許多危 險因子被認為和其發生有關,本實驗是以35位胰島素依賴型糖尿病人為對象。 其中20位女性,15位男性,平均年齡為21±4歲,此外以15位健康人為對照 組,而骨質密度是檢查右股骨頸及腰椎(L2 -4)二處。結果顯示:(1)糖尿病人此 二處之骨質密度明顯低於正常組。(2)糖尿病人之骨質損失和其糖尿病期長,血 糖控制較差及每日胰島素之注射量較大有關,而和病人之年紀、性別、體重、 血清鈣、鎂、磷、鹼性磷酸鹽濃度無關。總之,血糖控制之好壞對胰島素依賴 型糖尿病人骨質損失扮演著重要的角色。 |
英文摘要 | Osteoporosis is generally regarded to be a complication of insulin-dependent diabetes mellitus (IDDM). The significance of different risk factors for the development of bone loss in IDDM was evaluated in a cross sectional study of IDDM patients. Bone mineral density (BMD) in the right femur neck and 2nd to 4th lumbar vertebrae (L2-4) was measured by quantitative dual photon absorptiometry in 35 IDDM patients (20 females, 15 males; age 21.5±4.2 years) with those of a control group of 15 healthy subjects, matched for race, sex, age and body mass index (BMI). In diabetic patients, BMD of right femur neck and lumbar vertebrae was significantly lower than in controls (P<0.01) . The decrease in BMD in the diabetic group correlate well with duration of diabetes, quality of metabolic control ( fasting blood glucose, serum fructosamine and hemoglobin Alc levels: P<0.05) and daily insulin dosage (P=0.053), but not with patient's age, sex, BMI, serum calcium, phosphorus, alkaline phosphatase and intact parathyroid hormone (iPTH) levels (P>0.05). Thus, the quality of metabolic control are major factors in determining bone mineral content in insulin-dependent diabetic patients. |
