中文摘要 | 從1990年9月到1992年5月間,我們共收集了13隻罹患視神經病變之眼睛與13隻罹患黃斑部病變之眼睛,進行同步成型視覺激發波與成型視覺發波在患視視經病變之眼睛組中,有7隻眼P100延遲(這7隻眼之P100平均值為108.87msec;正常眼P100值為94.41±3.51msec),有5隻眼P100消失,1隻眼P100正常。它們的成型視網膜電圖,有8隻眼以正常之P50振幅但減弱或消失的N95表現。成型視覺激發波在罹患黃斑部病變之眼睛組中,有2隻眼P100延遲(平均P100值為103.59msec), 7隻眼p100消失,4隻眼P100正常。它們的成型視網膜電圖,有12隻眼睛皆沒有正常之P50振幅,即P50與N95同時減弱或消失。由這次研究,我們發現同步成型視覺激發波與成型視網膜電圖之記錄,與只記錄成型視覺激發波相比較,可以提高視神經病變與黃斑部病變之鑑別診斷率;前者由57.1%到100%,後者由42.9%到80%,因此,我們建議對於視交叉以前之視覺徑路障礙,擬做電氣生理學瞼查時,應同步測定成型視覺激發波與成型視網膜電圖,以提高診斷之正確性。 |
英文摘要 | Thirteen eyes of optic neuropathy and 13 eyes of maculopathy during Sep. 1990-May 1992 were studied by simultaneous pattern VEP (PVEP) and pattern ERG (PERG) recording. In optic neuropathy, the P100 of PVEP was delayed in 7 eyes (ave. 108.87 msec; normal 94.41±3.51 msec), absent in 5 eyes, normal in 1 eye. Their PERG showed normal P50 amplitude (either normal or delayed latency) but small or absent N95 in 8 eyes. In maculaopathy, the P100 of PVEP was delayed in 2eyes (ave. 103.59msec), absent in 7 eyes, normal in 4 eyes. Hwoever, their PERG shoed 12 eyes had small or absent P50. combined PVEP & PERG test, when compared to PVEP alone, increased the diagnostic value from 57.1% to 100% for optic neuropathy and form 42.9% to 80% for maculopathy in this study. Thus, simulataneous PVEP & PERG recording is highly recommended in evaluation of anterior visual pathway disorders. |
