中文摘要 | 站立平衡不良是巴金森氏患者的一大功能障礙表徵,這也使得巴金森氏 患者成為易於跌倒的高危險群之一。本研究的目的在分析、比較健康之中、老年 人與巴金森氏患者動態站立平衡之穩定度,以了解巴金森氏患者與健康之中、老 年人在動態站立平衡穩定度的差異,並提供未來臨床訓練之參考。本實驗共有二 十六名受試者,其中巴金森氏患者十三名(依Hoehn和Yahr之分期及修定之韋伯氏 評分表再區分為七名輕度,六名中、重度功能障礙)平均年齡六十四歲,以及十 三名年齡配對之健康中、老年人,無任何骨科或神經科等足以引起平衡不良之疾 病者,以測力板系統(Balance Master system)來評估受試者之動態平衡穩定度,包 括二種測試:(一)規律的左右、前後重心移動:以三種不同的移動速度三秒、二秒、 一秒作規律的左右或前後重心移動,評估重心移動距離及時間;(二) 穩定度極限: 評估八個不同方向的重心移動時間,準確度及身體晃動程度。結果顯示在穩定度 極限測試中,巴金森氏患者其重心往前方、右前方、左後方及左前方移動所需時 間,均明顯比健康中、老年人長(p<.05)。其中,中重度巴金森氏患者重心往右方、 左前方之移動時間又較健康之中、老年人為長(p<.05)。中、重度巴金森氏患者其 重心往右方、後方移動的時間也顯著地較輕度巴金森患者為長(p<.05)。然而,在 其他測試上,巴金森氏患者與健康之中、老年人並無明顯的差異。此可能由於參 與本實驗之巴金森氏患者其功能障礙程度較輕及受試者較少所致。雖然,本研究 結果顯示,重心移動時間(movement time)可做為區別巴金森氏患者與健康之中、 老人在動態站立平衡穩定度之一有效平衡參數。(中華物療誌1994:19(2)106-115) |
英文摘要 | The purpose of this study was to investigate and compare dynamic standing stability on the NeuroCom* Balance Master system in normal subjects and Parkinsonian (PD) patients. Thirteen PD patients, aged 43 to 77 years, and 13 age-matched normals participatedin the study. Seven patients were classified as mild and six as moderate disability sub-groupby Hoehn and Yahr staging and a modified Webster's rating scale. The dynamic stability wasquantitatively assessed on the Balance Master for all subjects. The assessments include (1)amplitude and time for each excursion during lateral and front-back rhythmic weight shifttests and (2) movement time, accuracy and body sway while tracking from central to eightperipheral targets during limits of stability test on a computer screen. The results of pairedt tests showed that the movement time taken from central to front, right-front, left-back, andleft-front target was significantly longer in PD group than normal group (p<.05). The results of independent t tests showed that the movement time taken to back target for patientswith moderate disability was significantly longer than mild disability (P<.05). The resultsof one way ANOVAs revealed that the movement time taken from central to right target forpatients with moderate disability was significantly longer than mild disability and normals(p<.05) and to left-front target for patients with moderate disability was significantly longerthan normals (p<.05). There were no significant differences between groups or among subgroups and normals in other dynamic variables.
