中文摘要 | 糖尿病病患罹患腎臟病變,最後導致末期腎臟衰竭而需要洗腎者,有逐漸增加的趨勢,許多研究指出微白蛋白尿(microalbuminuria),可作為偵測早期糖尿病腎病變的指標,本實驗的目的在於利用三天的飲食記錄,初步探討膳食中蛋白質等營養素的攝取量與糖尿病腎病變的相關性。以AlbuSure定性糖尿病患的微白蛋白尿。實驗組有15人微白蛋白尿呈正反應表示有腎病變,對照組有21人呈負反應顯示無腎病變,比較患者血清中膽固醇、三酸甘油脂、飯前、飯後血糖、尿素氮、肌酸酐等項目。評估營養素攝取量,採用連續三天的飲食記錄(3-day dietary records),並以電腦分析營養成份。結果顯示糖尿病發病年齡、理想體重百分比率、三酸甘油脂、飯前、飯後血糖兩組並無統計上的差異。但是糖尿病的痛齡中(p<0.05)、血清膽固醇(p<0.05)、尿素氮(p<0.05)、肌酸酐(P<0.01)有腎病變者顯著高於無腎病變者。無腎病變者比有腎病變者膳食中的蛋白質佔總熱量百分比率顯著為低(14.7 vs 17.3,P<0.05)、粗纖維則顯著為高(p<0.05),而碳水化合物與脂肪的攝取量相似,沒有差異。 |
英文摘要 | Diabetic patients with the complication of nephropathy become prevalent.Several studies in recent years have shown that microalbuminuria is a goodpredictor of clinical nephropathy. The purpose of the study was used 3-day-dietary-record to examine the difference of dietary intake among diabetic patientswith and without nephropathy. Patients were divided into two groups, 15 patients microalbumin positive (albuminuria≧20 mg/dl) indicated with nephropathy, 21 patient microalbumin negative (albuminuria < 20 mg/dl) indicated without nephropathy. We used 3-day-record-keeping technique to examine nutritional intake. Serum cholesterol, triglyceride, AC and PC blood glucose, urea nitrogen, creatinine were measured. The differences between diabetic patients with and without nephropathy wereanalyzed with t-test. Patients without nephropathy had higher daily intake ofcrude fiber than did patients with nephropathy. Also, Patients without nephropathy took a significantly lower proportion of their total daily calories as protein. |
