| 中文摘要 | 本研究以高雄醫學院小兒科54例年齡介於16天至180天之膽汁滯留症嬰兒為對象,其中包括新生兒肝炎患兒42名,膽道閉鎖患兒12名,另以36名年齡介於1個月至6個月之正常嬰兒做為對照組。利用酵素及電泳方法分析其血清脂蛋白中膽固醇之成份。結果如下:嬰兒膽汁滯留症和對照組比較,呈現(l)總膽固醇值有意羲昇高(p<0.05)。(2)高密度脂蛋白-膽固醇有意義下降(p<0.05)。(3)低密度脂蛋白-膽固醇有意羲昇高(p<0.05)。另外,在膽道閉鎖羣中有9例(75%)出現脂蛋白-X,而在新生兒肝炎羣中僅6例(14.3%)出現,呈統計上有意義之差別(p<0.001)。以脂蛋白-X的出現來診斷膽道閉鎖,其敏感性75%,特異性85.7%,正確率83.3%。 |
| 英文摘要 | It has been recognized that marked changes in serum lipids and lipoproteins are frequently related with liver disorders. And the cholestasis is accompanied by the appearance of abnormal lipoprotein-X (LP-X) in serum. Determination of LP-X provides a clinical test in differential diagnosis of obstructive and non-obstructive jaundice. The materials included 54 cases of infantile cholestasis (biliaiy atresia in 12 and neonatal hepatitis in 42) and 36 age-matched healthy infants as controls. An enzymatic method was performed for determination of serum cholesterol in each lipoprotein fraction which is separated by electrophoresis. The geometric mean value of HDL-cholesterol was lower in infantile cholestasis than in controls, 6.3±4.5 mg/dl vs 44.2±1.4 mg/dl (p<0.05). The geometric mean value of LDL-cholesterol was higher in infantile cholestasis than in controls, 143.1±1.5 mg/dl vs 110.2±1.2 mg/dl (p<0.05). The total lipoprotein-cholesterol was higher in infantile cholestasis than in controls, 206.9±65.1 mg/dl vs 169.7±23.7 mg/dl (p<0.05). LP-X was detected in 9 of 12 biliary atresia and 6 of 42 neonatal hepatitis (X^2=17.149, p<0.001). None of the controls has detectable LP-X. The sensitivity, specificity and accuracy of LP-X in diagnosis of biliaary atresia were 75%, 85.7% and 83.3% respectively. The results indicated that the serum lipoprotein-cholesterols in cholestasis were characterized by (a) a decreased concentration of HDL-cholesterol (b) an increased concentration of LDL-cholesterol (c) an increased concentration of total lipoprotein-cholesterols. And, the simple, quick enzymatic test for LP-X is of clinical use in diagnosis of biliaary atresia. |
