中文摘要 | 本文描述照顧一位70歲大腸癌女性病人面對首次居家化學治療之護理經驗。筆者在2010年3月17日至9月8日護理期間運用Gordon十一項功能性健康評估,發現個案有面對首次居家化療知識缺失、因接受化療引發潛在危險性感染及噁心嘔吐副作用、與訊息不足引起之焦慮及主要照顧者緊張等健康問題。筆者於門診照護期間建立良好的護病關係,藉由護理指導鼓勵個案參與並協助其面對首次經歷且頻繁的居家化療,提供以居家為基礎的化療照護,協助個案與案夫安心在家完成12次療程,成功推展以醫院為基礎的居家化療模式。希冀藉此護理經驗分享,鼓勵臨床護理人員協助癌症病人勇於接受門診居家化療,兼具治療與生活節律,提昇病人與家庭成員之生活品質。 |
英文摘要 | This case report described the nursing care process of helping a 70-year-old female who was diagnosed as colon cancer, and received the first time home chemotherapy. The authors used Gordon's 11 functional health patterns as a guide to identify and assess the patient problems. The following health problems were identified: anxiety due to a lack of knowledge on home chemotherapy, the management of the related side effects (e.g. nausea, vomiting, infections), and the primary caregiver's nervousness. By using professional knowledge, establishing good nurse-patient relationship, and encouraging the patient to face the challenge of receiving home chemotherapy, the authors helped the patient and her husband complete the twelve courses of the home chemotherapies. Furthermore, the authors implemented family centered nursing care, and then successfully promoted the hospital-based home chemotherapy model. By sharing the nursing care experience, the authors expect to encourage clinical nurses to help cancer patients receive home chemotherapy fearlessly, maintain a balance between their treatment and life, and then enhance the life quality of the patients and their family. |
