中文摘要 | 多重用藥,多發生於老年人,文獻顯示當病人的藥物使用越多,發生藥物交互作用的機率就會越大,藥物不良反應的發生率也會增加,造成醫療資源的浪費。本研究目的為針對門診病人用藥,分析其可能的藥物交互作用的現況。 本研究為回溯性研究,收集本院民國93年1至3月,處方數至少兩筆、處方天數大於等於14天之門診病人資料,進行藥物交互作用之比對。 本研究中發生可能的藥物交互作用次數共53,624次,其中發生可能的藥物交互作用者的平均年齡為58歲者男性的比例佔48%,平均處方藥物筆數為5.8筆。發生可能的藥物交互作用模式,以 C2 (moderate、probable) 發生的次數最多,共29,858次,佔所有藥物交互作用的55.7%。而嚴重度較高的藥物交互作用,等級A1-B1,共發生825次,佔1.5%。屬於禁忌 (A1-A3) 的藥物交互作用,有110次。其中以心血管用藥發生可能的藥物交互作用最多,佔47%,而又以 digoxin 與其他藥物發生可能的藥物交互作用次數最多。如果就醫療科別來分析,則以心臟內科發生的藥物交互作用次數最多,佔38%。 建議未來可以建立一個藥物交互作用的監測指標系統,並將它與病人的檢驗數值、藥品血中濃度等資料相關連。如此可以在臨床上幫助醫師,在面對藥物交互作用時,做更好的臨床決策。 |
英文摘要 | Polypharmacy is common in the elderly. It was suggested that the incidence rate of drug interaction increases as the item of medication increases, which may result in the increase of incidence rate of adverse reaction and health expenditures. The objective of this study is to analyze the patterns of potential drug interactions among the outpatients. This is a retrospective study. The drugs prescribed to outpatients in our hospital during January 2004 to March 2004 was screened for the pattern of potential drug interaction including severity, documentation and onset. In this study, 53624 potential drug interactions were identified. The mean age of the patients with potential drug interactions was 58 years, 48% of them were male, and the mean items of prescribed drug were 5.8. The most common of potential drug interaction pattern is grade C2 (moderate, probable) in 55.68% with 29858 times. The incidence rate of potential drug interaction of pattern A1-B1 is 1.5% (825 times). Among the contraindicated pattern of drug interaction (A1-A3, 110 times), the incidence rate of cardiovascular drug is the highest in 47%, and digoxin is the highest one among them. The highest rate (38%) of potential drug interaction was prescribed by the cardiologist among all medical specialties. Development of the key monitoring indicators system linked to the patient's laboratory profile for detection of drug-interactions is recommended. It will be helpful to the physician to make better clinical decision in patient with risk of drug-interaction. |
