中文摘要 | 研究背景:字詞聽辨測驗是語音聽力檢查的重要項目之一,可預估個體的語音聽辨表現,然而施測音量會影響字詞聽辨測驗的表現,但目前有關何種音量可得到最佳字詞聽辨得分(Maximum speech recognition ability, PB MAX)的研究並不多。研究目的:本研究以華語音素均衡測驗做為材料,分析不同施測音量測得的字詞聽辨得分與PB MAX的差異,除了探討這些施測音量的可施測率,也探究各音量測得的字詞聽辨得分與PB MAX之吻合率。研究方法:對象為28個中度至中重度感覺神經性聽損耳(分屬於19位女性,平均年齡51歲),施測情境有未助聽與助聽兩種,字詞聽辨的施測音量包括最舒適音量(most comfortable level, MCL)、最不舒適音量(uncomfortable level, UCL)、UCL-2 dB與下列感覺音量(sensation level, SL):25 dB SL、30 dB SL、35 dB SL、40 dB SL、45 dB SL等,此外,也以2 dB的級距建立字詞聽辨得分音量曲線(performance intensity functions, PIF),PB Max被界定曲線的最高點。研究結果:(1)無論是未助聽情境或助聽情境,40 dB SL、45 dB SL、UCL-2 dB的字詞聽辨得分與PB MAX都無顯著差異,即表現近似於PB MAX。(2)UCL-2 dB的可施測率與此音量測得的PB MAX吻合率,在各音量的比率都最高。研究結論:建議採取UCL-2 dB作為臨床PB MAX的施測音量,因為全部的受試耳都可以用此音量施測(可施測率100%),並且此音量的字詞聽辨得分與PB MAX的吻合率高於其他音量。 |
英文摘要 | Purposes: Word recognition test is one of the important test items in speech audiometry. It gives an estimation of subject's performance in speech recognition. The administrative intensities would affect the scores. However, not many studies investigated what intensity would yield the Maximum Phonetically Balanced speech recognition score, PB MAX. This study used Mandarin mono-syllabic PB words as test material and analyzed the differences between PB MAX and speech scores obtained at various intensities. Both the testability of various intensities for word recognition test and the percentage of agreement between speech score at various intensities and PB Max were investigated. Methods: Twenty-eight ears were investigated. They belonged to 19 female (average age = 51), who had moderate to moderately-severe sensorial neural hearing loss. Subjects were tested in unaided and aided conditions. Test intensities include the most comfortable level (MCL), uncomfortable level (UCL), UCL-2 dB, and the following sensation levels (SL): 25 dB SL, 30 dB SL, 35 dB SL, 40 dB SL, 45 dB SL. PB MAX was defined as the peak score in performance intensity function (PIF), which was established in a 2 dB-step procedure. Results: (1) PB scores at intensities of 40 dB SL, 45 dB SL, and UCL-2 dB were similar to PB MAX, in both unaided and aided conditions. (2) UCL-2 dB had the highest percentage both in testability among various intestines in administering speech recognition test, and in the agreement between the PB MAX and PB scores. Conclusions: It is recommended that UCL-2 dB be adopted as the intensity in administering speech recognition test, not only because it is testable at this intensity for all subjects but also because the PB score yielded at this intensity is very close to the PB MAX. |
