中文摘要 | 一隻三歲齡未絕育之雌性馬爾濟斯犬,因近期呈現運動不耐及食慾不佳之症狀,至地方動物醫院診斷出永存性動脈導管而被轉診至中興大學獸醫教學醫院。理學檢查於左心基處發現連續性第五級(V/VI)心雜音與股動脈強烈搏動(hyperkinetic pulse);放射線學檢查發現主動脈幹膨大及肺血管過度循環(overcirculation);心臟超音波學檢查發現左心房與左心室擴張、有一進入肺動脈幹持續噴射性擾流、輕微二尖瓣收縮期及肺動脈瓣舒張期擾流。綜合以上檢查結果臨床診斷為永存性動脈導管。以心導管螺旋彈簧絲圈栓塞術進行治療,手術將心導管由股動脈伸入動脈導管,放置1.5圈的螺絲彈簧絲圈於肺動脈端,再將其餘彈簧圈釋放於主動脈,形成栓塞進行治療。術後24小時於心臟超音波發現左心室容積減少,且已無擾流進入肺動脈幹,但仍有二尖瓣收縮期及肺動脈瓣舒張期擾流。術後第65天於心臟超音波學下已無顯示二尖瓣收縮期擾流,但仍有肺動脈瓣舒張期擾流。 |
英文摘要 | A three-year-old intact female Maltese dog had a month history of exercise intolerance and poor appetite. Patent ductus arteriousus was diagnosed at the local animal clinic and was referred to VMTH-NCHU. On physical examination, a grade VNI continuous murmur was audible over the left heart base and the femoral pulse was bounding. Thoracic radiographs demonstrated a prominent aortic bulge and hypervascular lung fields suggestive of overcirculation. Echocardiography demonstrated left heart dilatation and showed the presence of continuous turbulent flow entering main pulmonary artery, mild mitral valvular systolic and pulmonary valvular diastolic turbulent flow. The preliminary diagnosis was left to right shunting patent ductus arteriousus. Transcatheter helical coil occlusion was performed, catheter was inserted through the femoral artery and 1.5 loops of the coil were positioned in the pulmonary artery and the remainder of the coil in the ductus ampulla. Twenty-four hours after the operation, echocardiography demonstrated a decrease in the left heart volume and showed no sign of turbulent flow entering main pulmonary artery, but still presented with mitral regurgitation (MR) and pulmonary regurgitation (PR). On echocardiography 65 days later, MR was no longer detected but still presented with mild PR. Patients PR condition is still under careful surveillance. |
