中文摘要 | NM23基因是在1988年,由老鼠黑色素瘤K1735細胞株中,在高轉移細胞, NM23基因表現明顯減低而被鑑別出來,因而將NM23基因歸納為轉移抑制基因。NM23基因分為NM23-H1,NM23-H2,DR-NM23,NM23-H4。其中NM23-H1基因被研究出在有高轉移性的非口腔癌細胞中有較低的表現,但對口腔癌組織NM23之表現不甚暸解。本篇研究是收集台中榮民總醫院1983至1995年,口腔頰黏膜及舌鱗狀上皮細胞癌病患中,接受腫瘤廣泛切除和頸部淋巴結切除手術,且為歷資料和病理切片保存完整的106位病患。(男:女=101:5,年齡:27-82歲、平均50.5歲,頰黏膜55例,舌51例)。將腫瘤細胞用免疫組織化 學分析的方法,研究NM23-H1基因的表現。結果106位病患整體NM23-H1基因表現是增強的 (佔66%、共70位)。就性別、口腔鱗狀上皮細胞癌部位 (頰黏膜或舌)、腫瘤分化程度、有無遠端轉移,來分析NM23-H1基因表現的強弱,經x2檢定分析結果並無統計上有意義之區別(P>0.05)。但手術後頸部淋巴結經病理切片證實有頸部淋巴結轉移和沒有轉移的病患中,有轉移病患NM23-H1基因表現較低者的比例 (42.1%,57位中有24位) ,較沒轉移者高 (24.5%,49位中有12位);但未達到統計上之意義 (P=0.058)。本研究歸納出NM23-H1基因在頰黏膜及舌鱗狀上皮細胞癌中有較強的表現;但在有頸部淋巴結轉移的病患中,NM23-H1則有表現減低的情況 (P=0.058)。雖未達到統計學土之意義,就臨床上而言,頰黏膜及舌鱗狀土皮細胞癌細胞NM23-H1基因的表現有其潛在之可行性當作預測腫瘤頸部淋巴結轉移的指標,值得進一步探討。 |
英文摘要 | MN23 gene is identified from murine melanoma cell line in 1988. In its highly metastatic cell line (K1735), the expression of NM23 gene is reduced. The following studys recognized the NM23 gene as a metastatic suppressive gene and identified that reduced expression of NM23-H1 gene is related to highly metastatic potential. One hundred and six specimens of the buccal mucosa and tongue cancer (squamous cell carcinoma) were selected in Taichuang Veteran General Hospital, Taiwan. The specimens were divided according to genders (male, or female), sites (buccal mucosa, or tongue), tumor differentiated (well, or non-well), neck lymph nodes metastasis (yes, or no) and distant metastasis (yes, or no). The expression of NM23-Hl gene of these specimens were analyzed immunohistochemically. The results show that the expression of NM23-H1 gene is increased in these 106 cases. There are 70 cases (66%) positively staining. Chi square analysis shows that the expression of NM23-H1 gene is not statistically related to the genders (male, or female), sites (buccal mucosa, or tongue), tumor differentiated (well, or non-well), and distant metastasis (yes, or no) (P>0.05). While in those cases of neck lymph nodes metastasis, there is evidence of slightly reduced expression of NM23-H1 gene (P=0.058). There are 24 cases (42.1%) of negative staining in 57 cases which have neck lymph nodes metastasis compared to 12 cases (24.5%) of negative staining in 49 cases which have no neck lymph nodes metastasls. This study shows that the expression of NM23-H1 gene is increased in the buccal mucosa and tongue cancer, and in those cases of neck lymph nodes metastasis, there is evidence of slightly reduced expression of NM23-H1 gene but not statistic significant (P=0.058). |
