中文摘要 | 目的︰報告一例偏遠地區醫療資源匱乏,因輸血造成人類免疫缺乏病毒 (Human Immunodeficiency Virus,HIV) 感染的學童,討論長期其家庭及學校的心理適應和照顧。病例報告︰一位十四歲的國三男學生,在七歲時因車禍輸血,不幸感染人類免疫缺乏病毒。當時因消息曝光造成學校同學及鄰居的排斥,導致個案和家庭面對巨大的心理壓力。經由衛生署主導,整合省立澎湖醫院精神科及衛生局協助,以公衛護士擔任個案管理人的方式作長期的追蹤,協助其個人和家庭的身心適應。目前個案持續在某醫療中心定期接受抗HIV藥物控制,其學校和社會生活的適應良好。結論︰學童感染HIV會因其不同的認知發展過程、家庭因應模式、文化習慣等因素影響其身心適應。除監視其病程之外,亟需整合身心照會工作。兒童精神科醫師的介入,利用個別及家庭的心理治療以協助其生活適應有其必要。由於偏遠地區醫療資源不足,以公衛護士作為個案管理人的方式,對一種慢性而進行性疾病的掌控是值得的。 |
英文摘要 | Objective: To report a case of school-child with HIV infection and discuss about his psychological adjustment and model of long-term psychotherapeutic intervention. Case report: Six years ago a fourteen-year-old boy was found to have an HIV infection due to blood transfusion. He suffered social rejection from schoolmates and neighbours, he also faced significant psychological distress. The public-health nurse worked as case manager to help the patient and his family face their new social dilemma. Presently the patient continues to visit the medical center and receiving anti-HIV medication regularly. He has also adapted well to his new social life in school and at home. Conclusion: When a school-child contracts HIV, his cognitive development, the coping style of family and culture habits will influence his ability of psychological adaption. We need to monitor his medical progress, and the psychotherapeutic intervention by the case manager is also necessary especially in a rural area with inadequate psychiatric resources. |
