中文摘要 | 臨床上,目前對非增殖性糖尿病視網膜病變(NPDR)的病人,其中心視力變差的原 因還不清楚, 本篇研究的目的即是對此類病人做視網膜中心凹錐形細胞功能 (foveal cone function) 的研究, 我們藉分析 foveal cone electroretinogram 的變化, 來評估其外層 視網膜的功能是否正常, 並進一步探討其與中心視力喪失是否有關連。我們使用 dual-beam stimulator-ophthalmoscope 檢測三組□ normal subjects (22 eyes) □ NPDR with CSME (24 eyes) □ NPDR without CSME (45 eyes) 的 foveal cone electroretinograms。我們 統計分析發現 NPDR with CSME 這組的平均振幅明顯比 normal subjects (t-test, p<0.05) 和 NPDR without CSME (t-test, p<0.05) 低, 但分析 NPDR with CSME 和 NPDR without CSME 這二組的平均振幅, 並無統計學上的差異。 至於分析三組的反應延遲時間 (implicit time), 彼此並無統計學上的差異存在。 從 foveal cone electroretinograms 的結果得知平均振幅在 NPDR 病人有降低的現象,尤其是在 NPDR with CSME 的病人。此種 不正常的反應顯示外層視網膜功能有不正常的現象,而這可能就是告成 NPDR 病人中心視力 減退的原因。 |
英文摘要 | Purpose:To evaluate foveal cone function in patients with nonproliferative diabetic retinopathy (NPDR), with and without clinically significant macular edema (CSME). Methods:Focal electroretinograms (ERGs) were elicited with a stimulator-ophthalmoscope from normal subjects (22 eyes), NPDR without CSME (45 eyes), and NPDR with CSME (24 eyes). The 42 Hz with the flicker stimulus was centered on the fovea and subtended 5 degrees within a steady 20 degrees white annular surround. Results:Mean amplitude was lower in eyes with CSME and eyes without CSME, compared with that in normal eyes, Eyes with CSME had significantly lower amplitude than normal eyes (t-test, p<0.05) and eyes without CSME (t-test, p<0.05). However, mean amplitude in eyes without CSME was not significantly lower than normal subjects. There was no statistical difference of implicit time between normal subjects and NPDR patients. Conclusions:Our investigation suggests that outer retinal dysfunction in eyes with NPDR particularly in the presence of CSME. The abnormalities in foveal cone response in NPDR patients may be the etiology of loss of vision in diabetic patients. |
