中文摘要 | 自發性蜘蛛網膜下腔出血,一般是以腦動脈瘤破裂為主。目前診斷方法以電腦斷層腦部攝影可檢查蜘蛛網膜下腔出血,緊急情況可用電腦斷層血管攝影,可快速查出動脈瘤位置,若仍不能確定,則可用傳統腦血管攝影檢查來證實。手術時間一般以早期手術較佳,即動脈瘤破裂三天內為主,其優點是可減少再破裂機會,減少血管痙攣,降低水腦發生機會,同時日後之照顧較簡單容易。手術以夾住動脈瘤為主。 |
英文摘要 | Rupture of cerebral aneurysm is the main issue of spontaneous subarachnoid hemorrhage (SAH). At present time, computed tomography (CT) is the cornerstone for subarachnoid hemorrhage. In emergency, computed tomographic angiography (CTA) may early detect the size & location of aneurysm. It is the safe & effective method for screening of aneurysm. Traditional cerebral angiography may provide the final decision of aneurysm, if the CTA is not confirmed. Early operation, within 3 days of rupture of aneurysm, is advocated. There are four benefits of early operation, decrease the rate of ruptures of aneurysm, decrease rate of vasospasm, decrease the rate of hydrocephalus and provide the care uncomplicated Clipping of aneurysm is the major procedure for rupture of aneurysm. |
