中文摘要 | 臨床上多根牙的牙齒,若有根叉侵犯(furcation involvement)的問題,會影響 到它的預後。傳統上以牙根整平翻瓣手術( flap open curettage )或牙根切除術( root resection )來解決問題,但這些方法只能使根叉侵犯的情況達到穩定狀態,而沒有再生的 效果。近年來,由於再生手術的進步,學者們嘗試克服根叉侵犯的問題。目前用在根叉侵犯 治療的牙周再生手術中,通常在下顎二級或上顎頰側二級根叉侵犯的治療效果較好,使用不 吸收膜和可吸收膜的治療效果差異不大;另外,生長因子則是目前最具潛力的引導牙周再生 技術,不過生長因子的相關研究仍然太少,因此進一步的研究是必要的。本文介紹和比較各 種牙周再生手術。 |
英文摘要 | There is always questionable prognosis if furcation involvement occurs to multirooted teeth. Traditionally, flap open curettage or root resection was performed to try to solve this problem. However, these methods could only achieve a stable condition of the furcation involvement teeth, and had no regenerative effect. Recently, due to the improvement of regenerative procedures, scholars had studied hard to try to overcome the problem of furcation involvement. This paper introduces and compares several kinds of regenerative procedures by way of arranging past studies. At present, no statistically significant differences have been observed between the regenerative periodontal therapies with absorbable and non-absorbable membranes. But there was significant improvement when the therapies were used in mandibular class Ⅱ or maxillary buccal class Ⅱ furcation involvement teeth. On the other hand, growth factor was the most potential regenerative periodontal therapy. However, studies about growth factor are few. So more advanced studies are necessary. |
