中文摘要 | 臨床上,約1%的腹部電腦斷層檢查會意外發現大於1公分的腎上腺腫瘤,雖然其中絕大多數是非功能性腎上腺腫瘤,仍造成許多困擾。 良性腎上腺腫瘤常含有豐富的細胞內脂質,可以做為和其它腎上腺腫瘤區別的根據。在做磁振影像時,因水和脂質在磁場中向量旋轉速度不同會產生訊號的差異:如於兩者向量位於相同方向時取像(in-phase image),兩者訊號會加成;於兩者向量位於相反方向時取像(out-phase image),兩者訊號會相減而降低。因此在兩者同向與反向時各取一組影像,比較訊號是否有差異即可判定其中是否含有脂質。本研究以1.5T的磁振造影配上體表線圈,及相同成像條件的水和脂質同向與反向的快速多層次梯度迴訊磁振影像(FMPSPGR),來檢視判斷腎上腺腫瘤是否含有脂質而為良性腺瘤。因使用體表線圈,接收的訊號較高,取像範圍可縮小為20x20∼24x24公分,取像厚度可縮為0.6公分,每一voxel相當於4.9∼7.0立方釐米,為全身型線圈的1/4∼1/6,可減少測量小腫瘤訊號變化時的邊界誤差;且影像的品質良好,解析度高。又因成像條件相同,所以可以用簡單的計算目測法來判斷腫瘤在同向與反向的影像上訊號是否有有意義的差距,簡化診斷的手續,增加臨床實用性。研究中收集了85位病患共92個病灶,分別測量脾臟及腎上腺腫瘤在同向與反向影像上的訊號強度並加以比較。結果證實腎上腺非腺瘤和含脂質極少的脾臟的訊號強度在同相及反相磁振影像都沒有有意義的差別,而腎上腺腺瘤則在反相磁振影像有非常明顯訊號降低的差異(差異平均值為53.37)。隨所選定的差異值的不同,本研究中的檢查方式對小於6公分的腎上腺腫瘤是否為腺瘤的偵測力最佳可達89.85%的敏感度和96.65%的專一性。如用目視辨別,則可達到91.3%的敏感度和96.65%的專一性。證實這種磁振檢查方式對鑑別腎上腺腺瘤與非腺瘤有很高專一性與偵測率,且十分簡便、快速及安全之輔助檢查。 |
英文摘要 | Differentiation between benign adrenal adenomas and other adrenal lesions is clinically necessary. Based on the theories that adrenal adenomas can be differentiated from other adrenal lesions by their high intracellular lipid content and that on out-phase MR images, there is sional cancellation in lesions containing both water and lipid, we design this study to test the efficacy of chemical shift MR images obtained with surface coil and in- and out- phase fast multiplanar spoiled GRASS sequences using otherwise the same imaging parameters. Ninety-two adrenal tumors (less than 6 cm in size) in 85 patients were collected. Among them, there were 70 adenomas and 22 other tumors. The signal differences of the spleen and the adrenal lesions on out-versus in- phase images were measured and calculated. Any perceptible signal differences in lesions on visual inspection were also recorded. The results showed that there was no significant signal changes in the spleen and other tuomrs using calculation on out-and in- phase images. There was an obvious signal decline in adenomas on out-phase images. The sensitivity and specificity for differentiating adenomas and other tuomrs using calculation could reach 89.85% and 95.65%, respectively. The sensitivity and specificity of differential diagnosis using visual inspection could also reach 91.30% and 95.65%. Hence, MR images using a surface coil and paired in- and out-phase images can provide a easy, fast and safe adjuvant method to differentiate adrenal lesions with high sensitivity. |
