中文摘要 | 本報告敘述一位面積32.5%重度燙傷的老人,住進燒傷中心,左面臨身體功能、 角色及收入等種種的失落,引起的起伏不定情緒,不但影響整個醫療計劃,同時 在缺乏學習自我照顧動機和信心下、也增加了住院治療天數。筆者為個案之主護 護士,在護理個案期間,以Orem's自我照顧系統理@論評估其需求,確立皮膚完 整性受損、營養狀況改變、現存性感染、活動無耐力、焦慮和知識缺失等六個健 康問題。過程中,學習應用老年失落的護理,與個案建立良好的互動互信關係, 使其情緒適當發洩,亦藉由家屬之關"u及支持,誘發學習動機,肯定自我能力與 重建希型,並擬出院計刮,指導及促使其學習受傷後的自我照顧。個案出院後經 電話訪談,瞭解已可自行下床走動、接電話及按時服藥等,且與家屑維持良性之 互動。亦經由護理此個案,體會到照護老年人不僅需注重生理上的問題,同時要 關心和協助老人在心理方面需求問題的適應和支持。 |
英文摘要 | This study case involved a patient, an old woman, who was burned, had 32.5 ﹪T.B.S.A. and was admitted into the ICU of Burns. The patient suffered agonies from physical dysfunction, deprivation of character and finance due to burns which therefore caused feelings of loss. Her feeling of loss and lack of motivation and confidence in learning self-care obstructed the medical treatment process and increased her length of stay. During the patient' s admission, her needs were evaluated, and the impaired skin integrity, altered nutrition, incidence of infection, activity intolerance, anxiety and lack of knowledge were identified based on the Orem' s theory by nursing staff. Meanwhile, I learned how to utilize aging-loss nursing to establish good activity. And trust with each other. And let the patient's emotion out of her mind in time. And the patient was Even motivated to learn self-care techniques and rebuilt her confidence to plan out-hospital programs to teach herself after-injury care with her family's care and support. After the patient was discharged from the hospital, a follow up telephone call was initiated to understand that the patient was making progress and was interacting well with her family. Through this case study, conclusions were perceived that nursing services for elder are not only cares of physical treatment but also cares involving the understanding of the patients' suffering of both body and mind. |
