中文摘要 | Indapamide 是一種利尿劑,對高血壓患者有治療效果,但對移動性血壓的變化則 鮮有報告。 本研究針對 15 位輕度至中度本態性高血壓的病人, 於服用每天 2.5mg Indapamide 前後作移動性血壓記錄( ABPM ), 分析其血壓變化,並檢驗血液生化及副作 用。 所用的儀器為 Del Mar Avionics 1990 型。 結果發現 Indapamide 治療 8 週後,24 小時收縮壓平均值、舒張壓平均值均有意義地降低, (收縮壓降 13mmHg,舒張壓降 5mmHg ),收縮期與舒張期高血壓的頻度亦均減少,前者減 71%,後者 52%,收縮期所受的影響比 舒張期大,而白天與夜間的變化相等。反之,心跳數、血壓變異度及日夜血壓差則無啥改變 。血壓的晝夜節律性亦不受影響。血液生化檢查中,除血鉀稍降與尿酸稍增外均無變化。有 3 個病人因肌肉酸痛需補充鉀鹽,此外無明顯副作用。基於此,我們認為 Indapamide 每天 2.5mg 為對國人輕至中度高血壓有效的治療藥物。 |
英文摘要 | Indapamide, an indoline derivative of chlorosulphonamide, possesses potent hypotensive properties as assesssed by casual blood pressure (BP) measurement. To assess the responses of ambulatory BP to oral indapamide in Chinese hypertensive patients, monotherapy at a single dose of 2.5 mg daily was performed on 15 patients with mild to moderate essential hypertension. There were 9 males and 6 females, ages ranging from 26 to 65 years old (mean±SD being 44±9 years). 24-hour ambulatory BP was measured before and 8 weeks after treatment, using a Del Mar Avionics Model 1990 Pressurometer Ⅳ system. Under indapamide therapy, there were significant decreases in the total measurements of systolic BP average (a decrease of 13 mmHg), diastolic BP average (a decrease of 5 mmHg), frequency of systolic hypertension (71% reduction) and frequency of diastolic hypertension (52% reduction). BP in the daytime and the nighttime were equally reduced. Heart rate, variability of BP and difference in BP between daytime and nighttime didn't change under treatment. Indapamide didn't interfere with the circadian rhythm of BP. Plasma sugar, cholesterol, triglyceride, blood urea nitrogen and creatinine showed no statistically significant change. Serum potassium lowered from 412±0.36 mmol/1 to 3.51±0.54 mmol/1, and uric acid elevated from 5.79±1.56 mg/dl to 6.71±1.17 mg/dl. All the patients had no discomfort attributed to indapamide except 3 patients who needed oral potassium supplement due to weakness and muscle cramps. This study supports the effectiveness of indapamide in the treatment of Chinese patients with mild to moderate hypertension. |
