中文摘要 | 循環性全靜脈營養施予方式被認為可防止因使用持續性全靜脈營養而引發的肝膽病變。本實驗的目的是比較持續與循環性兩種全靜脈營養施予方式對大白鼠代謝作用、身體組成及肝功能之影響。雄性Wistar大白鼠(∼275克重)經靜脈插管後分兩組施予相等體積、熱量及氮量之全靜脈營養輸液(第0天約 26 kcal;第1至7天約62 kcal),24H組於24小時內(2.4-2.6 ml TPN輸液/小時),而18H組於18小時內(3.0-3.2 ml TPN輸液/小時)施予一天之量。另包括經靜脈插管後施予生理食鹽水並由口攝食之對照組(Sham)。大白鼠於第7天犧牲(9:00至12:00)。Shan、24H及18H各組七天內之體重增加量及飼養效率無顯著差異。18H組身體組成之水份百分比顯著低於,但脂肪百分比顯著高於24H組與Sham組(p<0.05)。24H組之肝臟膽固醇含量顯著高於18H組與Sham組。24H組及18H組之血清葡萄糖、胰島素、白蛋白、全蛋白、尿素氮、三酸甘油酯、膽固醇、麩氨酸草醯乙酸轉胺�t、麩氨酸丙酮酸轉胺�t及膽紅素濃度並無顯著差異。本實驗結果顯示施予持續及循環性全靜脈營養方式對大白鼠代謝作用及肝臟功能試驗之影響無顯著差異,然而循環性全靜脈營養方式易造成大白鼠體內脂肪的堆積。 |
英文摘要 | It has been suggested that cyclic infusion of total parenteral nutrition (TPN) solution can prevent the hepatobiliary complications induced by continuous infusion. The aim of this study was to investigate the effects of cyclic infusion of TPN solution on the metabolic responses, body composition and liver function using a rat model. Male Wistar rats (∼275 g) underwent catheter placement (day 0) were infused with equal volumes of isocaloric and isonitrogenous TPN solutions (day 0: 26 kcal, day 1 to day 7: 62 kcal) for 24 hours (24H, 2.4-2.6 ml TPN solution/hour) or 18 hours (18H, 3.0-3.2 ml TPN solution/hour) a day. An orally fed group with catheter placement and saline infusion was included (Sham). There were no significant differences in body weight gain (day 0 to day 7) or feed efficiency among 24H, 18H and Sham groups. The percentage of carcass water was significantly lower, whereas that of carcass lipid was significantly higher in 18H group than in 24H and Sham groups (P<0.05). The hepatic content of cholesterol (mg/g liver) was significantly higher in 24H group than in 18H group. Serum concentrations of glucose, albumin, triglyceride, cholesterol, GOT, GPT, bilirubin, as well as tissue weights were not significantly different between 24H and 18H groups. In summary, even though our results did not show the significant differences in metabolic responses and liver function tests between continuous and cyclic TPN infusions, animals infused with cyclic parenteral nutrition were tended to accumulate fat in the body. |
