英文摘要 | Fixation problem of the tibial component due to fibrous tissue formation at the prosthesis-bone interface accounts for most failures in uncemented total knee arthroplasty. Micromotion of the prosthetic components prohibits bony ingrowth and induces fibrous tissue formation. The stem of the tibial tray provides temporary mechanical stability of the component. The permanent stability of the tibial component depends on the bony ingrowth onto the porous surface of the prosthesis. To investigate the potential benefit of tricalcium phosphate on the fixation of tibial prosthesis, a prospective clinical study was conducted. This study consisted of 61 TKA, which was divided into three groups, 20 cemented TKAs, 21 uncemented TKAs and 20 uncemented TKAs with tricalcium phosphate. The results in this study are based on forty seven patients with 12 plus months follow-up. TKA with tricalcium phosphate showed a faster rate of pain improvement. However, there was no statistical difference in pain rating among the three study groups. There was a statistically significant difference of functional scores between the cemented TKA and uncemented TKA. However, there was no statistical difference in knee scores among the study groups. The cemented TKA showed higher incidence of radiolucency which had increased three times from six months to twelve plus months. It appeared that tricalcium phosphate had no significantly positive or negative impact on the overall results of total knee arthroplasty in the early clinical experiences. |
