中文摘要 | 目的︰查理士•波內症候群(Charles Bonnet Syndrome)是用來描述發生於心智正常的個體,單獨患有視幻覺,未合併有其他形式之幻覺及妄想的現象。本文利用一例個案討論其神經生理學現象與致病機轉之關係。病例報告︰66歲已婚女性,自視力衰退4年後,單獨發生複雜之視幻覺。其系列檢查僅單光子電腦斷層攝影(SPECT)顯示左側視覺皮質區血流量減少。在于以抗精神病藥物(Sulpiride 200 mg╱day)治療後,症狀獲得緩解。結論︰查理士•波內症候群之致病機轉假說,目前普遍認為是因感覺刺激貧乏,中樞抑制機轉無法正常運作,致使視幻覺產生。因此治療主要為增加外界刺激,藥物治療之效果仍有爭議。本病例除週邊性視力減弱外,中樞初級視覺皮質區之血液灌流亦有減低,支持感覺刺激貧乏之假說。 |
英文摘要 | Objective: Charles Bonnet syndrome describes a psychological condition in which isolated visual hallucinations occur in the absence of other modalities of hallucination or associated delusional behavior. The etiology and pathophysiology of Charles Bonnet syndrome is not well understood. We reported a case and discussed its pathophysiological phenomenon and the possible pathogenesis. Case Report: A 66 year-old female experienced isolated, vivid, and complex visual hailucinations after long standing diminished visual acuity. Single positron emission computerized tomography (SPECT) revealed hypo-perfusion over the left visual cortex. Her symptoms inmproved under Sulpiride therapy (200 mg/day). Conclusion: Currently, the prevailing hypothesis about its pathophy-siology is the disinhibition of visual cortical activities after sensory deprivation which results in 'disinhibited' activities of the higher order visual association cortical areas, thus producing visual hallucinations. Accordingly, principles of treatment consist of increasing sensory stimulation, administration of neuroleptic agents and/or anticonvulsants. How-ever, there is still controversy regarding various modes of treatment. |
