英文摘要 | Weber's syndrome caused by ventral midbrain lesion is characterized by ipsilateral third cranial nerve palsy and contralateral hemiparesis. To date, reports in the literature concerning Weber's syndrome associated with the midbrain paresis of horizontal and vertical gaze are rare. A 59-year-old woman suddenly developed right third cranial nerve palsy, left upgaze palsy, left abduction palsy, transient visual hallucination, and left hemiparesis. Intact Bell's phenomenon, preserved left vertical and horizontal oculocephalic reflex suggested the gaze palsy in left eye was supranuclear in nature. Magnetic resonance imaging (MRI) revealed infarction in the right paramedian midbrain-diencephalic junction, right cerebral peduncle, and right midbrain tegmentum. It has been speculated that lesions affecting the rostral interstitial nucleus of medial longitudinal fasciculus (riMLF), the mesencephalic reticular formation, and the substantia nigra pars reticulata are responsible for the gaze and psychiatric dysfunctions. This case demonstrates neurologic heterogeneity of midbrain infarction. |
