中文摘要 | 國軍部隊訓練將手榴彈投擲列為五項戰技之一,共軍亦列在六大技術之中,可見手榴彈投擲訓練的重要性。根據國外文獻資料報導,發現手榴彈投擲性骨折只有少數病例,但國內趙尚良及蕭開平二位學者卻分別提出了129及30個病例,充份顯示過去國內軍陣中手榴彈投擲性骨折有偏高的情形。近年來,國軍部隊訓練用之手榴彈型式雖有趨於統一,重量亦減輕,在訓練時仍不時有因投擲手榴彈造成肱骨骨折之情況,如此嚴重影響我國軍戰力及部隊訓練之成效,為了增進部隊訓練成效及維護戰力,深入探討其原因及機轉是當務之急。本計劃以二年期間收集臺灣地區陸軍各總醫院手榴彈投擲性骨折病例,並追蹤統計四組新兵在新兵訓練中心的訓練期間有無手榴彈投擲性骨折之發生。依臺灣北、中、南、東四大區的骨折病患比例可知其區域分佈以南區為最,另外採用變異數統計分析法(ANOVA)作肱骨骨折病患與正常組之比較,所得結果發現肱骨骨折發生於中三分之一處者,身高最高且最重,其體格較一般人魁悟,擷抗張力較強,與其肌肉擷抗失調造成骨折發生有關。對於預防官兵在訓練期因投擲手榴彈而造成骨折,則應注意投擲的訓練方式,並作好投擲訓練前的熱身運動,如此能減少國軍發生手榴彈投擲性骨折的機會,對增進部隊訓練效果,保持國軍戰力,均有莫大的助益。 |
英文摘要 | A series of 195 clinical cases in which spiral fractures of the humerus had been sustained during hand-grenade throwing were reviewed. Normal body characteristics of 2186 young men and hand-grenade throwers with humeral fracture were collected. Examination of the roentogenograms from the clinical cases reveal that the fracture is of an external spiral fracture type occurring during an external rotation of the throwing arm. The hand-grenade thrower's fracture is consistently localized in one anatomical area, i.e. the distal one-third of the humerus (79.7%) with shorter personal (height=169.62 ± 0.53 cm,n=94) with no statistical difference with control. The height of throwers with hand-grenade-induced fracture over middle and upper third of humerus are 174.17 ± 1.24 cm (n=15) and 172.00 ± 2.68 cm (n=9), respectively. Correlativity between handgrenade thrower's fracture of upper and middle third of humerus with taller individuals suggests that a long arm of long status may contribute to the shift of the fracture. |
