| 英文摘要 | The incidence of obstructive sleep apnea syndrome in chronic renal failure has been reported to be 41-63%. In our previous study up to 67% of uremic patients in Taiwan had obstructive sleep apnea syndrome. To determine the effect of dialysis on obstructive sleep apneas in uremic patients, we studied 13 uremic patients complicated by obstructive sleep apnea syndrome. All patients participated in an overnight sleep study using polysomnography (Grass 78) before and two weeks and six months to one year after dialysis. In a previous study, we discovered that RDI (respiratory disturbance index) and DEF (desaturation event frequency) were correlated with the severity of obstructive sleep apnea syndrome in uremia. Effective treatment of sleep apnea syndrome is defined as a drop in RDI or DEF of at least 50 percent. For patients on short term dialysis (two weeks after dialysis), drops of DEF were noted in 54% (7/13) of patients, but only 15% (2/13) of patients had a drop in DEF of at least 50 percent. Drops of RDI were noted in 62% (8/13) of patients, but only 31% (4/13) of patients had a drop in RDI of at least 50 percent. For patients on chronic dialysis (six months to one year after dialysis) RDI was corrected in 80% (8/10) of patients, but only 30% (3/10) had a correction of at least 50 percent. DEF was corrected in 60% (6/10) of patients, but only 30%(3/10) of patients had a correction of at least 50 percent. However the RDI and DEF did not change significantly before and after dialysis treatment. The initial results showed some beneficial effects after chronic dialysis for obstructive sleep apnea syndrome in uremic patients, but the results were not statistically significant. A larger number of patients and a longer period of study are necessary to evaluate the effect of dialysis on obstructive sleep apnea in patients with uremia. |
