中文摘要 | 由1994至1995年兩年中有615名軍人,因暴露於武器噪音造成耳鳴或聽力喪失,至 本部求診。研究記錄基本資料、病史、症狀,並進行理學檢查,純音聽力檢查,及鼓室圖檢 查,分別探討左右耳聽力閾值,耳鳴及年齡間之關係。結果顯示噪音性聽力喪失軍人求診人 數,有逐漸增加的趨勢。其中 95% 患者兩側氣導平均聽力閾值在 0.25、0.5、1、2 kHz 都 在正常範圍(小於 25 dBHL ),在 4 及 8 kHz 則有明顯聽力喪失;1、2、4、8 kHz 有兩 耳間非對稱性聽力閾值,其中 4 kHz 處兩耳聽力平均差值最大,為 4.5 dBHL。兩耳皆有耳 鳴者, 其聽力喪失程度在 4 及 8 kHz 處較無耳鳴者或單側耳鳴者嚴重, 平均閾值為 45 dBHL;單側耳鳴者,耳鳴側聽力喪失程度在 4 及 8 kHz 處較無耳鳴側嚴重,此現象又以左 側耳鳴者明顯,兩耳平均聽力差值可達 20 dBHL。年齡愈高,其高頻率聽力喪失程度愈嚴重 , 本研究中大於 50 歲者, 在 4 及 8 kHz 之平均聽力閾值可達 70 dBHL。 (中耳醫誌 1997;32:14-21 ) |
英文摘要 | In the past two years (1994-1995),the percentage of soliders with noise-induced hearing loss increased gradually in our department. Through their medical history and various check-ups, this study intends to investigate noise-induced hearing loss in 615 soliders who had exposed to variety of weapons noise. It is found that even though the air conduction threshold levels of both ears rarely exceed 25 dBHL at 0.25、 0.5、 1、 2 kHz in about 95 percentage of cases, but have deteriorated obviously at 4 and 8 kHz. Interaurally asymmetrical hearing-threshold is noted at 1、 2、 4 and 8 kHz, and is significant higher in left ear at 4 kHz. The averaged hearing-threshold difference between the two ears is 4.5 dBHL at 4 kHz. The hearing loss at 4 and 8 kHz is higher in ears with bilaterally noise-induced tinnitus than those with unilateral tinnitus or without tinnitus. The averaged hearing-threshold can be up to 45 dBHL in ears with bilateral tinnitus. For soliders who suffered from unilateral tinnitus, the averaged hearing-threshold of hte tinnitus ear at 4 and 8 kHz is worser than those of the non-tinnitus ear. This finding is apparent in only left noise-induced tinnitus; however, for these ears, the hearing-thresh-old difference between the two ears can be up to 20 dBHL. This study also indicates that as the soliders get older, the degree of noise-induced hearing loss increases at 4 and 8 kHz. For the group that are older than 50 years old, the averaged hearing-threshold can be up to 70 dBHL. |
