中文摘要 | 焦慮是手術病人常見的問題,它不僅會引起病人手術前的情緒反應,也會影響手術後身體功能恢復,甚至延長住院天數,而有系統的護理指導可有效減緩病人焦慮程度,以降低焦慮對手術之影響。本研究目的為瞭解有系統護理指導於緩解不同手術病人焦慮的效果。研究設計採用前實驗法,研究對象包含接受開心手術、經導管電氣切割術,與白內障摘除並植入人工水晶體術等三類病人,合計150位。依不同手術病人的特性,設計手冊指導、團體觀看錄影帶、團體觀看錄影帶加團體討論、及個別口頭指導等四種實驗處置。以「情境與特質焦慮量表」做為評量病人焦慮程度的工具,以評價護理指導之成效。研究結果發現:各科病人對於手術的焦慮反應有顯著之不同,以心臟內科接受經導管電氣切割術治療病的焦慮程度最高,四種護理指導方式能有效減輕病人手術前的焦慮程度,且四種護理指導方式之效果是相類似。四種護理指導方式中,以個別口頭指導方式能顯著地降低接受經導管電氣切割術治療病人的焦慮程度,而團體觀看錄影帶方式最能節省護理人力成本。此外,團體觀看錄影帶、團體觀看錄影帶加團體討論、及個別口頭指導等三種指導方式,皆能夠增加病人對手術相關知識。本研究不但評價不同護理指導方式的成效,更驗證護理指導對手術病人的重要性,凸顯護理人員的獨特角色與功能。同時藉由研究結果也瞭解護理指導所耗費的護理人力成本,可提供未來在臨床實務中執行護理指導的參考。 |
英文摘要 | Anxiety may arouse the negative emotional responses of patients before surgery. This may be related to patient's wound healing, physical function, recovery and delayed bedside activities. It may even be associated with surgery complication. All of these may result in prolonged hospitalization, hence, ways for medicating anxiety related to surgery are pivotal for care. The purpose of this study is to evaluate the effect of systematic nursing instruction on patients undergoing different types of surgery. Pre-experimental study design was used in this study. One hundred and fifty patients that had undergone either open heart surgery. transcatheter electric ablation, and extracapsular cataract extraction were invited as research participants. The experimental methods included pamphlet aided instruction., video aided group instruction, video aided group instruction. And discussion, and individual oral instruction. These were designed in accordance with each patient's different surgery type. The research instruments were based on the trait and situation anxiety scale as welled the surgery related knowledge scale. The research results support the hypothesis that systematic nursing instruction can reduce anxiety levels of patients undergoing surgery effectively. The anxiety level of patients with different surgery varied. Patients with transcatheter electric ablation had the highest anxiety level. The effects of four different methods showed no significant difference. Yet, Individual oral instruction helped significantly reduce the high anxiety level of patients with transcatheter electric ablation. It terms of cost-effect benefit video aided group instruction was the most effective way to help with anxiety in patients. Also, surgery related knowledge scores could be increased significantly by added video aided group instruction, video aided group instruction and discussion, and individual oral instruction. The analysis of anxiety effects and cost-benefits of four different instruction methods gave the nursing administers important references to arrange improved nursing instruction in their clinical practice. |
