中文摘要 | 膝關節是荷重關節,若稍有病變常影響到病人的日常生活至鉅,所以在骨科門診 中因退化性膝關節炎症狀而求診的病人為數不少。 一般而言,在病情還未達到需要施行脛骨高位切骨術或人工膝關節置換術等適應條件時,於 保守治療方面,除了藥物及復健物理治療之外,還可利用楔形足底鞋的穿著,對膝關節對位 ( alignment )做間接矯正。 本研究利用生物力學之觀點,採用楔形足底鞋以改善膝關節 壓力的負荷分佈。 由接受楔形足底鞋治療的臨床病人 15 位,共 26 個關節, 利用 8 度的楔形足底板黏貼於 病人的鞋底,期望改變膝關節壓力的負荷分佈。 在治療過程中,藉臨床評估,X 光學及靜力學的分析做研究,楔形足底鞋的穿著確能使下肢 機械軸直立化並減少 1 度至 4 度, 脛骨內高丘之壓力 F2 及外側副韌帶( lateral collateral ligament )與髂脛束( iliotibial band )的張力 P 值的和平均減少 65.4 牛頓。在臨床方面,病人的疼痛及關節水腫均有顯著改善。 |
英文摘要 | Knss joint is an important weight-bearing joint in the human body. Osteoarthritic knee can cause great discomfort in our daily activity, furthermore, increasing number of patients are being treated annually in the orthopedic clinic due to osteoarthritis of the knee. In advanced, and symptomatic knee OA (osteoarthritis), surgical intervention with high tibial osteotomy or total knee arthroplasty may be the better choice of treatment. However, in the early stage of the knee OA, conservative treatment with rest, medication, and rehabilitation can also relieve patients' symptoms and signs. Nevertheless, corrective wedge shoe support may also decrease the degree of varus deformity of the knee joint; thus, relieving patients' symptoms and signs. The purpose of this study is to evaluate the effect of the corrective wedge shoe support in redistributing the joint pressure of the knee joint. Twenty-six knee joints in fifteen patients were treated conservatively with the 8°corrective wedge shoe support. During the therapy, clinical assessment, X-ray study, and biomechanical analysis were used to evaluate the effect of the corrective wedge shoe support. After the therapy, the mechanical axis decreased 1°-4°, and the joint reaction force in the medial knee joint decreased 65.4N. Clinically, symptoms of the osteoarthritis also improved significantly. |
