| 中文摘要 | 為了瞭解紅辣椒素敏感之感覺神經在逼尿肌收縮力扮演之角色,我們在 大白鼠活體進行膀胱內灌注,並利用大白鼠離體膀胱進行離體實驗,同時也進行 離體膀胱肌肉之收縮力研究 (人及大白鼠)。在活體之大白鼠膀胱灌注低濃度之 紅辣椒素顯示會有興奮作用,但在高濃度之下則在興奮作用之後出現抑制逼尿肌 收縮的現象。在表面灌注紅辣椒素之膀胱的離體全膀胱實驗中,以及離體之肌肉 條實驗中,顯示只有在1mM紅辣椒素濃度下,才能達到明顯抑制逼尿肌收縮力的 效果(電刺激及bethanechol刺激)。在人類的膀胱肌肉條實驗中,紅辣椒素可引發 微弱的收縮,而且與劑量相關。但也要達到1mM才能達到抑制逼尿肌收縮的效 果。這個初步研究結果顯示,紅辣椒素可以使感覺神經末稍釋放出一些神經多性 狀,這些多性狀會促進逼尿肌收縮,並且對經由神經傳導的反射性排尿有影響。 缺乏這些神經多胜�韞i能會減低逼尿肌之收縮力,甚至抑制反射性排尿。但是一 定要在高濃度之下這個效果才能達到。(慈濟醫學1995;7:167-178) |
| 英文摘要 | In order to understand how capsaicin affects detrusor contractility in rat and human urinarybladders, three studies were undertaken. In the first study, capsaicin was administered intravesically toin vivo rats ; in the second, capsaicin was administered to in vitro rat bladders; and in the third,capsaicin was administered to in vitro human bladders. Intravesical administration of lowconcentrations of capsaicin exerted an excitatory effect on detrusor contractility in in vivo rat urinarybladders, whereas intravesical administration of high concentrations of capsaicin had an inhibitoryeffect. In the isolatedwhole rof bladder study and in the in vitro rat bladder muscle strip study, a highconcentration of at least ImM was necessary for achieving an inhibitory effect under either electrical orbethanechol stimulation. In the human bladder muscle strip study, the effect of capsaicin was moreevident and dose-dependent. The results of these studies show that capsaicin releases neuropeptides inboth rat and human bladders. It has been hypothesized that the release of these neuropeptides mightfacilitate normal detrusor contractility as well as reflexic micturition whereas a lack of theseneuropeptides might inhibit normal detrusor contractility and reflexic micturition.
