| 中文摘要 | 為了明瞭視神經炎病人中,視網膜神經纖維層的解剖形態變化,特別進 行此項研究。以視神經頭雷射掃描儀(Topss,LDT)檢查視神經炎病人25人,平均年 齡28.5歲,其中20眼視力恢復至正常1.0,18眼視力未完全恢復,12眼是臨床上未 發病眼,平均追蹤時間2年8個月。 檢查結果發現:不論是視力恢復眼或是未恢復眼壓神經纖維層的高度偏差(Height Variation)、等高線調制(Contour Modulation)以及上側神經纖維(Superior Nerve Fiber Layer)的高度都較正常組有低下的現象;而未恢復眼的下側神經纖維高度 (0.180mm比較對照組(0.254 mm)也有明顯低下的現象;至於這二組患眼的鼻側和顳 側的神經纖維層高度和對照組之間也有統計學上的差異。 未恢復眼的上側神經纖維層高度(0.142 mm)比恢復眼(0.225mm)有顯看統計學上 低下和損害的現象。至於臨床上單側視神經炎病人中,未患病眼的上、下側神經 纖維層高度雖然較正常組低下,但是未達到統計學上的顯看差異。 本研究發現,以視神經雷射掃描儀可以提供視神經乳頭周圍網膜神經纖維層極具 價值的解剖形態定量記錄,並可進一步藉以追蹤了解疾病的痛程發展和變化。本 報告首次對於視神經炎病眼做相對神經纖維層高度的分析和研究,未來希望能對 視覺功能、治療效果與神經纖維形態的相關性做更深入的探討。 |
| 英文摘要 | In order to further our understanding of anatomical & structural changes of nerve fiber layer in optic neuritis, the following study is conducted. Using Topographic Scanning System (Topss, LTD), 25 patients (mean age: 28.5 yrs) of optic neuritis were examined. Among the 38 affected eyes, 20 eyesregained normal vision (20/20). The average follow-up period is 2 years and 8 months. The results of our examination revealed that, nomatter regained normal vision or not, all affected eyesshowed decreased Height Variation and ContourModulation of nerve fiber layer, as compared withnormal controls. The height of superior nerve fiberlayer was also decreased In the eyes without fullrecovery, the height of inferior nerve fiber layer (0.180mm) was significantly lower than controls (0.254mm). The height of superior nerve fiber layer in eyes without full recovery (0.142 mm) was statistically significant lower than those who fully recovered(0.225 mm). In those clinically unilateral affectedpatients, the height of superior and inferior nerve fiberlayers of unaffected eyes were also lower than normalcontrols, but statistically insignificant.. This study showed that Topographic ScanningSystem can provide us with quantifiable data regarding anatomical & structural features ofparapapillary nerve fiber layer, which can be invaluably helpful in the understanding of diseaseprocess and clinical follow-up. We are lookingforward to conduct further investigations concerningthe association between visual functions, treatmentefficacies and nerve fiber layer configuration. |
