中文摘要 | 收集分析38例(男16女22)急性單一症狀性視神經炎 (acutemonosymptomatic optic neuritis,AMON)患者核磁共振及電腦斷層攝影之檢查 結果。病人平均年齡為32歲,平均追蹤時間為一年四個月。25(6例)患者之電腦斷 層攝影有異常發現,均為患眼之視神經擴大;33.33(7例)患者之核磁共振攝影於其 中樞神經系統(central nervous system,CNS)可見斑塊狀病變(plaque lesions),分布為 視交叉一例、視路徑(optic track)一例、視放射(optic radiation)四例、腦幹一例、脊 髓二例。三位病人於追蹤期間繼發其它症狀,經神經內科診斷為多發性硬化症 (multiple sclerosis,MS)。核磁共振攝影對急性單一症狀性視神經炎患者之中樞神經 系統斑塊狀病變有較高的檢出率,並可為多發性硬化症之診斷參考。 |
英文摘要 | It is well-known that patients with acutemonosymptomatic optic neuritis (AMON) and silentlesions in the central nervous system (CNS) have ahighly increased risk of developing multiple sclerosis(MS). To compare the sensitivities of magneticresonance imagiing (MRI) and computed tomography(CT) in detecting these CNS abnormalities, 38patients with AMON from September 1990 to August1994 were included in this study. MRI proved to bea more sensitive tool than CT, with 33.33 of patients showed plaques in optic nerve, chiasm, optic tract oroptic radiation. The ratio is lower than those reportedin western countries. Though 3 patients withabnormal initial MRI developed clinical definite MSduring follow up, one of our patients had fluctuateMRI-detectable lesions without change in theirclinical status. The prognostic implications of MRIare supported by the experience of the Optic NeuritisStudy Group, but should be applied with caution. |
