| 中文摘要 | 開心手術當中病患皆仰賴低體溫及體外循環技術來維持心搏停止狀態下的血流灌注,以保存各器官的功能。區域性器官的血流灌注受到血液黏稠度的影響甚巨,而血比容、血纖維蛋白原及溫度為調節血液黏稠度變化的最重要因素。本研究觀察了台大醫院20例接受開心手術病患,在體外循環期間其血液黏調度的變化,擬探討該變化的型態。我們在不同時間抽取血液樣本,加溫至攝氏37度後,以Silenus viscometer測定其黏稠度。結果顯示:在體外循環期間,各病患的全血黏稠度變化並無一致性,而血漿黏稠度及血比容25%之血液黏調度的變化不大。我們認為,新近所應用之人工心肺設備一膜式氧合器及體外循環技術,對於血液黏稠度並未見造成具統計學上意義的影響。 |
| 英文摘要 | Patients undergoing open heart surgery depend upon hypothermia and cardiopulmonary bypass (CPB) to maintain organ perfusion during cardiac arrest. However, regional organ perfusion is greatly influenced by blood viscosity. In practice, hematocrit, plasma fibrinogen concentration and temperature are the most important factors regulating blood viscosity. Changes in blood viscosity during intracardiac operations with CPB and membrane oxygenators were studied in 20 adult patients at National Taiwan University Hospital. Blood samples were withdrawn to assess the rheologic properties by measuring whole blood and plasma viscosity with a Silenus cone-on-plate viscometer. The results showed neither uniform changes of whole blood viscosity among the patients during CPB, nor statistically significant changes of plasma viscosity and blood viscosity with constantly adjusted hematocrit at 25%. It was concluded that, with the application of the new type of membrane oxygenator and improved CPB techniques, little change of blood viscosity can be found. |
