中文摘要 | 民國73年7、8月間,三軍總醫院在連續的16位開心手術病人中,以隨機取樣之方式,使用氫氣乙基澱粉(HES)與人體血液自蛋白(ALB)於體外循環中。兩組之人數相等,皆為8例。填加HES或ALB於體外循環之目的在增加體外循環時血液中之膠質壓以避免手術後大量之水分積存在組織間隙中。使用ALB雖然最合乎生理需要但其來源不易且價錢昂貴;HES相對地價錢便宜甚多且很容易取得。唯一HES未被廣泛採用的原因乃是它"可能"有抑制凝血之副作用,但有些報告則指出該副作用除非在大量使用HES才會發生。本研究乃使用16 cc/Kg之6% HES溶液於體外循環中,其劑量所能使血中膠質壓升高之程度與4 cc/Kg之25% ALB相當,故將此兩者對凝血、出血量、肝功能、腎功能及肺功能之影響做一比較。研究結果顯示兩者在前述之各項之比較時並無顯著之差異;實驗僅發現體外循環本身可使血小板及凝血纖維原之數量降低,使凝血酵素原時間(PT)及部分血栓形成激素時間(PTT)延長,並使SGOT及SGPT升高。由此可見若在血液白蛋白不易大量取得之情況下,使用氫氧乙基凝粉於體外循環是一可行的辦法。 |
英文摘要 | Colloids have been used in extracorporeal circulation to increase oncotic pressure and to prevent tissue swelling following cardiac surgery. Human albumin is an ideal colloid, but its supply is limited and hence expensive. Hydroxyethyl starch is much cheaper and has an equal ability with albumin to increase blood oncotic pressure, but its effect on blood coagulation is still unknown. Sixteen open-heart cases were randomly divided into two groups in which human albumin or hydroxyethyl starch was used in extracorporeal circulation. Blood coagulation, blood loss, amount of blood transfusion, urinary output, liver function, renal function, and pulmonary function have been studied in these cases. From analysis of the data, it is concluded that (1) human albumin and hydroxyethyl starch, when used to prime the extracorporeal circulation, have similar effects on coagulation, blood loss, amount of blood transfusion, liver function, renal function, and pulmonary function; (2) extracorporeal circulation itself may interefere with blood coagulation by decreasing platelet count and fibrinogen, as well as by increasing prothrombin time and partial thromboplastin time, and (3) with its minimal disturbance of coagulation and its much lower price, hydroxyethyl starch is an ideal substitute for human albumin when human albumin is not available. |
