中文摘要 | 本研究的主要目的是比較巴金森氏病患與正常人之呼吸功能,同時探討 其影響呼吸肌肌力酌可能因素。口位輕度巴金森氏病患(Hoe-nandYah,I及TI級1 為PI組。10位中度巴金森病患(HoehnandYahrII)及IV級1為PZ組。8位年齡、身高、 體重相當者為對照組C。代表呼吸肌力的最大吸氣壓1Pimax.與最大吐氣壓 1PEmax1足用壓JJ儀(pressuremeter1測量。肺功能的用力呼吸容積(八,C1與第一秒 用力呼氣量1FEv,1足用肺量計仁pirometer1測量。結果顯示:(1)Pg組的Pimax與 PEmax分別為50.8與50.6公分水柱,明顯低於PI組(其PImax與PEmax分別為94.4 與89.6公分水柱)及C組(其Plmax與PEmax分別為115.3與100.9公分水)(p<0.05);(2)P2 組的FVC與FEV1分別明顯低於P1組與C組(p<0.05);(3)Plmax及PEmax分別與FVC及 FEV1明顯相關(p<0.05);(4)疾病的嚴重度及運動狀況與吸肌肌力明顯 相關 (p<0.01。所以,本研究結果顯示輕度巴金森氏病患的呼吸功能並未明顯低於正常 人。然而,疾病的嚴重度及運動的有無是影響呼吸肌肌力之重要因素。(中華物 療誌1994;19(2)99-105) |
英文摘要 | The major purposes of this study were to compare the respiratory function betweenParkinsonian patients and normal subjects, and also to examine the possible factors affecting the respiratory muscle strength. Eleven patients in Hoehn and Yahr stage I and II (withmild disability) were classified as group PI. Ten patients in Hoehn and Yahr stage III andIV (with moderate disability) were classified as group P2. Eight age-, height-, and weightmatched normal subjects were chosen as control group C. The respiratory function wasassessed in a sitting position. The results indicated that:(1) the maximal inspirator)' pressure (Pimax) of inspirator)' muscles and the maximal expiratory pressure (PEmax) of expiratory muscles in group P2 were 50.8 cmHft and 50.6cmHfl, respectively, which weresignificantly (p< 0.05) lower than that in group PI (94.4 cmHO and 89.6 cmH,0, respectively) and group C (115.3 cmH2O and 100.9 cmH 0, respectively); (2) the forced vitalcapacity (FVC) and forced expiratory volume in one second (FEV1) in group P2 were 1.8Land 1.3 L, respectively, which were significantly (p<0.05) tower than that in group PI andgroup C; (3) Pimax and PEmax correlated significantly (p<0.05) with the FVC and FEV ,respectively: (4) the severity of disease and the level of exercise were highly related to Pimax (p<0.01). These findings suggest that the respiratory function is not significantly reduced in Parkinsonian patients with mild disability. The severity of disease and the level ofexercise may contribute to tile decrease of respiratory muscle strength in Parkinsonianpatients. (JPTA ROC 1994;19(2):99-105) |
