中文摘要 | 本研究探討"脈動性電磁場效應電刺激"應用於"頑固性骨折不癒合"的臨床治療成效。患者自最後一次手術至開始接受電刺激的骨折不癒合期平均達18個月(範固自10至25個月不等)。其中13位男性,7位女性,平均年齡為37歲(涵蓋17至69歲不等)。在電刺激治療之初,有7例為併發骨髓炎。本研究中每位患者於電刺激治療之前,至少均接受過兩次以上的手術。依骨折不癒合的部位分佈:腔骨12例;股骨6例:舟狀腕骨2例。"脈動性電磁場效應"來自於特定波形的脈波。其間包含波寬為5毫秒之方形不對稱陣波,其頻率為15赫,輸出至平行治療線圈之電壓則為10伏。骨骼受到此效應產生之誘導電壓約為1至1.5毫伏強度。線圈隨著石膏固定於患部,每天於睡眠時使用6至8小時,全程需費8至16週。結果其中90%頑固性骨折不癒合經電刺激達到癒合。最初7例併發骨髓炎之患者也有5位癒合。平均每位患者之電刺激治療全程需時9.9週。由本研究結果顯示:"脈動性電磁場效應電刺激"是"頑固性骨折不癒合"治療上的一項有力選擇。 |
英文摘要 | A prospective study was performed of the first twenty consecutive patients who had fractures with recalcitrant nonunion, treated by pulsing electromagnetic field (PEMF) stimulation. The average duration from the time of last operation to start of PEMF treatment was 18 months (range, 10 to 25). The 13 male and 7 female patients were of an average age of 37 years (range, 17 to 69 years). Seven patients had osteomyelitis at the time of treatment. Forty-one operative procedures had been previously performed, or an average of more than two procedures per patient. The tibial shaft was the site of nonunion in 12 patients; the femur, in 6; the scaphoid bone, in 2. A specific pulse of current was fed to the coils from a generator which drives the coils at 10 Volts. The generator produced a five-millisecond burst of quasi-rectangular asymmetrical pulses repeated at 15 Hertz. The driving voltage to the coils was set to produce 1.0 to 1.5 millivolts of induced current in the bone. The magnets were mounted on the plaster, and current was applied for 6 to 8 hours daily during sleep for periods from 8 to 16 weeks. Eighteen (90%) of the nonunions achieved solid osseous union. Five of the seven patients who had an infected nonunion achieved solid union. The average duration of treatment with PEMF was 9.9 weeks (range, 8 to 16 weeks). This study demonstrated that PEMF method is a good alternative in the treatment of recalcitrant nonunion. |
