中文摘要 | 本研究利用73個接受完全靜脈營養的患者在使用前及連續三週間測其血漿中的白蛋白、前白蛋白及攜鐵蛋白,由其變化,瞭解短期(< 3週)使用完全靜脈營養後,其血漿蛋白質的變化。 很明顯地,本研究群在治療前的三種蛋白質均較控制群呈現有意義的偏低。在靜脈營養後,前白蛋白只需一週時間就有明顯地上升,然而白蛋白及攜鐵蛋白到第三週末仍未有意義的上升。依其臨床結果可分死亡群及存活群,在存活群中血漿前白蛋白及攜鐵蛋白均呈有意義的上升,血漿前白蛋白只需一週,而攜鐵蛋白卻需三週的營養治療才能呈現有意義上升。相反地,在死亡群中三種蛋白質皆無明顯的變化。另外,在比較存活群及死亡群中,可發現連續性的測試血漿前白蛋白及攜鐵蛋白可提供一個好的預後指標。 在本研究中,血漿前白蛋白與攜鐵蛋白的測定呈現良好的相關性(r=0.7172);同時並顯示隨著營養治療的時間愈長其相關性愈好。另外,在三種血漿蛋白質中,白蛋白及攜鐵蛋白對營養治療的反應似乎較前白蛋白為遲緩。因此,在監測短期間完全靜脈營養(< 3週) )的療效中,血漿前白蛋白比白蛋白及攜鐵蛋白二者可提供更好的營養指標。 |
英文摘要 | The responses of plasma proteins to a short-term total parenteral nutrition (< 3 weeks) were investigated. We measured plasma albumin, transferrin and prealbumin in the beginning and three consecutive weeks of nutritional repletion in seventy-three patients. Baseline values of albumin, transferrin and prealbumin in these patients were significantly lower than that in healthy volunteers. After repletion, significant rise in prealbumin required only a week of nutritional support, while that in albumin and transferrin had shown no significant changes until the end of third week in overall cases. However, a significant changes in transferrin had shown by the end of third week in the survival group. In contrast, no response of these proteins to nutritional repletion had observed in the mortality group. Moreover, serial measurements of prealbumin and transferrin may provide good prognostic indicators in the course of total parenteral nutrition (TPN). In our study, values of plasma prealbumin have a good correlation with those of plasma transferrin (r=0.7172), and it seems more correlative with a time course. Of the three proteins, the rates of response to nutritional support in plasma albumin and transferrin seem slower than that in plasma prealbumin. Hence plasma prealbumin is a more valuable indicator other than plasma transferrin and albumin in monitoring the acute nutritional changes in a short-term TPN (< 3 weeks). |
