| 中文摘要 | 運動訓練是改善慢性阻塞性肺疾患者症狀的治療中不可缺少的一環,經 過一段時間的訓練後,病人的運動耐力會增加,最大攝氧量會提高,運動技巧會 改善。雖然我們不十分清楚到底是經由什麼機轉來改善病人的運動能力,也不知 道長期下來是否會有副作用,但是可以用有限的資料及我們的臨床敏感度來做指 引。經驗告訴我們簡單的測試及訓練方法一樣安全而有效,不一定需要昂貴的儀 器和設備,應廣為推行到慢性阻塞性肺疾患者身上。在目前給予患者的運動處 方,在早期或較輕微的患者可以比照同年齡的正常人,然後根據病人的症狀來修 改運動強度和目標,而較嚴重的患者將會是受症狀限制,最好是採用特定的運動 方式慢慢增加運動時間和強度,且應測試氧不飽和情形以決定是否需加氧氣治 療。至於有肺心症(cor pulmonale),左心室衰竭、體重明顯減輕或有不協調胸腹運 動的患者,則應該非常小心的給予小量運動或甚至不要運動。 |
| 英文摘要 | Exercise training is a powerful tool forimproving the symptoms of patients withChronic Obstructive Pulmonary Disease.Important and significant improvements do occurin exercise endurance after exercise trainingprogram. However, we are hampered by notknowing precisely how it works, and knowingvirtually little of its longterm adverse effects,especially on the heart, pulmonary circulation,respiratory muscles and joints. Guidelines forthe use of exercise training must thereforebe governed by the clinical sensibilities, aswell as the availing data. Experience has shownthat simple forms of exercise training andtesting are effective and safe. These can bedone without the use of complicated andexpensive equipments. Most patients shouldnot only be encouraged to remain physicallyactive to the limits of their tolerance, but should also be started on appropriate physicalconditioning programs, depending on theirdisability. For the present, one can giveexercise prescription for early COPD patientsin the same way as for normal subjects ofsimilar age, modifying the intensity and goalsto the patient's symptoms. Patients with moreadvanced disease will be symptom-limited, andshould have exercise training which attemptsto improve the performance of specific tasks.Arterial desaturation should always be measuredin the laboratory in patients with more advanceddisease to determine the need for supplementaloxygen. Patients with cor pulmonale, failingleft ventricle, lean body wieght loss ordysynchronous thoracoabdominal motion shouldbe exercise very cautiously or not at all. |
