中文摘要 | 本研究的目的在比較正常人和慢性阻塞性肺疾者在極限運動和低限運 動在穩定狀態時的心肺功能反應,三種低限運動之運動強度分別是最大攝氧量 (V0�痂ax的50%、60%及70%)。11名男性慢性阻塞性肺疾患者及9名年齡、體型、 活動度相近的正常人接受腳踏車肌功器的極限運動測試,最大運動量時的各項心 肺功能反應均予以偵測、記錄,並在運動後5分鐘檢查乳酸濃度。在1週內,每名 受試者再接受3段10分鐘的低限運動,在3個研究的低限運動量前受試者都先以10 瓦的量運動5分鐘,每股低限運動前受試者都予以充份的休息,低限運動時的心 肺功能反應都以絕對數值及與該測量參數在最大運動量值的百分比表示。結果顯 示病人極限運動時的工作量、攝氧量、通氣量、潮氣量、呼吸商和5分鐘的乳酸 濃度均較正常組低,其餘參數兩組沒有顯著差異。在各低限運動時,病人有較高 比率的最大通氣量和潮氣量(p<0.05),三種不同運動強度之心肺功能反應除在50 和60VO�痂ax時之呼吸頻率,以及在60和70VO�痂ax的潮氣量無明顯差異外,其 餘都有統計意義上的差別。同時這三個運動量下的攝氧量和通氣量佔其最大值的 比率相近,此點或可有助於慢性阻塞性肺疾患者的運動處方。 |
英文摘要 | The purpose of this study was to compare the exercise responses in normal subjects andsubjects with chronic obstructive pulmonary diseases (COPD). Eleven stable COPD patients fromoutpatient department and 9 persons with no known cardiopulmonary diseases served as the subjects of the study. Cardiopulmonary responses to maximal workload during an incremental exercise test and submaximal steady-state exercises of three intensities, 50, 60, and 70 VO maxrespectively, were studied. No differences in resting or preexercise data between the groups werefound. Subjects with COPD attained comparable breathing frequency (Fb), heart rate (HR), oxygen pulse, ventilatory equivalent for O�畝nd CO�� (VE/VO�� VE/VCO��), and O�� pulse with thenormal controls, but significantly lower workload (WL), oxygen consumption (VO��), minute ventilation (VE), tidal volume (Vt), and respiratory exchange ratio (RER) at maximal exercise. Fiveminute postexercise lactate level was also significantly lower in patient group. The absolute values of all these parameters required for 50, 60, and 70 VO�痂ax in COPD patients were notsignificantly different from the values in normal subjects. However, VE as a percentage of achievedmaximal values was significantly higher in these three exercise intensities in COPD patients.Minute ventilation for intensities of 50, 60, and 70 VO�痂ax were found to be 51.1±5.9,62.2±10.6, and 70.3±9.0 of the patients' maximal VE. |
