中文摘要 | α-胎兒蛋白(簡稱AFP)是一種分子量7萬左右的醣蛋白質。以往用免疫沉澱法發現在出生後二、三個月之內即無法自血液中測知其存在;但是利用具有高敏感度的放射性免疫分析法,吾人已可證明它在嬰兒時期會維持在一個相當高的濃度並且持續一段長時間,這個現象常常導致此時期內α-胎兒蛋白判讀上的困擾。為了訂出一個正常範圍以作為日後臨床應用的基準,台大醫院小兒科收集了231位正常中國嬰兒的血液並用放射性免疫分析法測定血清α-胎兒蛋白值;其中包括17位早產兒與214位一天到一歲大的足月產嬰兒。利用各種過歸模式分析其資料,發現可將它們的變化歸納成一個過歸公式:logY=7.397-2.622log(X+10);其中X為出生天數,Y為以ng/ml為單位的α-胎兒蛋白清值;相關係數(r)為-0.947,標準差為0.381。再根據此公式,即可以算出一歲之內正常嬰兒血清值的95%預測帶。一般而言,此正常範圍的變化在出生後一個月之內最為明顯而呈急劇下陣,以後下降速率則隨著年齡的增加而逐漸變緩,直到一歲時嬰兒正常值的上限才趨近於成人的標準(20ng/ml)。所以在半對數圖上面呈現一種曲線下降的變化型態。這種表現顯然不同於一些可分泌α-胎兒蛋白之腫瘤經手術完全切除後的血清值下降變化,可見在嬰兒時期,α-胎兒蛋白的減少不僅受到其自然分解機轉的影響;據此可以推論在出生以後,嬰兒可能仍會持續合成一些較成人為多量的α-胎兒蛋白。對於早產兒而言,其血清α-胎兒蛋白值一般要比足月新生兒為高,所以在比對正常範圍時必須考慮到是否有早產的現象存在。目前仍然不知道這種因懷孕週數不同而造成的差異會持續多久,但至少在出生後數週之內不應把早產兒的數值拿來跟足月嬰兒的正常值範圍直接比較。 |
英文摘要 | Serum alpha-fetoprotein (AFP) levels were measured in 17 premature babies and 214 normal Chinese infants, including 43 full-term neonates and 171 infants between 1 and 12 months old. The normal ranges of AFP in this age period were constructed by regression analysis. A 95% prediction band, based on the equation log Y=7.397-2.6221og(X+10) (X:age in days, Y:AFP level in ng/ml; r=-0.947, S.D.=0.381), was then obtained. The decline rate of AFP was found to be most rapid during the first few months of life and decrease successively thereafter until 12 months of age when the estimated upper limit approximated closely the adult level (20 ng/ml). The premature neonates tended to have higher AFP values, a fact to be considered in data interpretation. |
