| 中文摘要 | 爲瞭解學童之尿液生化在形成結石的危險因子中所扮演的角色,本研究在對6-18歲間之正常學童1,072人及62位成人教職員之在校第一次,8小時及24小時尿液中鈣,尿酸,無機磷,鎂,鈉,鉀,蛋白質及滲透度和pH的分析中發現,除18歲者之24小時尿鈣,尿酸濃度及pH值(23.8±12.5mg/dl),(29.6±7.2mg/dl)及(6.29±0.50),均比12歲者之尿鈣,尿酸濃度及pH值(9.0±6.0mg/dl, 10.5±2.4mg/dl 及 5.94±0.23),與8歲者之尿鈣,尿酸濃度及pH值(8.8±5.9mg/dl, 19.0±5.2mg/dl及5.86±0.53)有明顯的增加外(P<0.001),其24小時尿鈉濃度(159±37mEq/L)也比12歲與8歲者之尿鈉濃度(139±40mEq/L, 107±35mEq/L)也有校明顯的增加(P<0.05)。且在16~18歲間之單一次(在校第一次)尿中鈣濃度及8小時尿中鈣濃度也均比之其他年齡層有明顯的增加(P<0.001)。其他各項生化濃度及尿量在各年齡層間並無統計學上的差異(P>0.05)。我們假定尿中高濃度之鈣,尿酸與鈉及排泄量似乎仍爲我們所統計的近30年間年齡在16~20歲間結石病例的增加之危險因子。但是否與飲食狀況,環境,遺傳,飲水量甚至於尿液中抑制結石形成的抑制因素(inhibitors)等有關,仍有待更進一步之分析與查證。 |
| 英文摘要 | Renal calculi are much less in children than in adults, and high urinary concentrations of calcium, uric acid, urine pH and a low volume of urine are known to be risk factors. To better understand the urinary risk factors of forming stones in school children, a quantitative study of urinary biochemical constituents was performed. Except for a concentration of calcium in spot morning urine, the calcium concentrations of 8-hour urines and 24-hour urines were much higher in the age group 16 to 18 years than in other age groups (P<0.001), the urinary concentrations of uric acid, sodium, osmolarlity and urine pH in age group of 16 to 18 years were also significantly higher than those of other age groups (P<0.05). The results shown are quite compatible with increasing renal stone formation beginning at age 17, as analyzed on 9,936 stone formers treated at the National Taiwan University Hospital in the past 30 years. This study seems to indicate that the ages of 16 to 18 years may be the physiological risk period for urinary stone formation. As to the role of concentrations of urinary inhibitors (citrate, mucopolysaccharide, pyrophosphate etc) in school children, especially in the age group of 16 to 18 years, further investigation is needed. |
