中文摘要 | 對靜脈注射腎盂攝影術,腎騙不能顯影的病人而言,超音波檢查是一種簡單、迅速、便宜、且非侵犯性之檢查。不論腎臟機能如何,診斷性超音波檢查對尿路阻塞是一個很敏銳的指標,它能偵測出病人腎臟實質中已經無機能的部位。使用超音波檢查我們便可知道腎臟是否存在,假使存在,便可知道腎臟之大小、形狀和內部的結構。當超音波檢查在腎臟窩內不能發現腎臟時,必須讓病人仰臥,仔細搜索骨盆,以期發現異位腎臟。 於過去兩年半中,本人以超音波掃描來研究尿路病變,尤其是研究單側腎臟未能顯影的43個病人,這些單側腎臟未能顯影的病人中,最常見之原因是尿路阻塞,有19個病人佔44.2%。第二個常見之原因為腎臟結核,有13個病人佔30.2%,這些病人的腎臟有不同的超音波圖式,如不規則之中心回音,輕度腎水腫,往往也可看到整個腎臟有散布之回音。有時腎臟之大小正常而在髓狀回音中有"無回音"之中心圈。在腎臟窩內不能發現腎臟者有5個病人佔11.6%,其中4個為腎臟發育不全,1個為異位腎臟,至於實質性腫塊導致腎臟未能顯影者並不多見,有5個病人佔11.6%。 |
英文摘要 | Ultrasonography provides an easy, noninvasive, rapid, and inexpensive means of evaluating patients when excretory urography shows a nonvisualizing kidney. Diagnostic ultrasouod, since it is independeot of renal fuoction, is ao eveo more seositive indicator of urinary obstruction, detecting those cases where no fuoctioning renal pareochyma is preseot. Using ultrasouod, one can determine whether a kidney is present, and if so it can provide information regarding it size,shape and internal architecture; when a kidney is not identified in the renal fossa on ultrasonogram, a careful search of the pelvis with the patient in supine position should be performed to rule out renal ectopia. The purpose of this studies is to present my experience with B-mode ultrasound scanning in the investigation of the lesions of urinary tract with particular reference of unilateral of nonvisualizing kidney (43 cases) during the last two and half years. Of these, 19 (44.2 perrcent) were urinary obstruction, which is the most common cause of unilateral nonvisualizing kidney. The next most common is renal tuberculosis, 13 patients (30.2 percent) have demoostrated a varied ultrasound pattern, as irregular of central echo aod mild hydrooephrosis. Usually a diffuse echo pattern though out the kidney was seen. Sometimes the kidney is normal sized, the medullary echo pattern has a ceotral echoless core. Among the 5 patients (11.6 percent) no kidney is identified in the renal fossa on ultrasound, 4 patients of renal agenesis, and one patieot of renal ectopia were discovered. Solid mass lesions causing total nonfunction of the kidney are rare (5 cases, 11.6%). |
