中文摘要 | 二十世紀生物醫學科學有驚人進步,醫藥衛生行政與計劃並沒跟上進步,從現有醫學科學觀點來看,行生醫療機構所提供的服務與應該提供服務之間有很大的差距,這個差距就表現在民眾是否能善用醫療、保健服務,為了解這方面的問題,本研究以分層隨機集體取樣(Stratified random cluster sampling)方法在臺北市十六區中每區隨機選一里,再在此里中隨機選一鄰最後共得16鄰574戶逼樣本戶以擬妥之問卷登門訪問然後將所得資料計算(一)向醫師求診機率(Probability of physician visitation) (二)向醫師求診的平均頻次(Mean number of physician visits) (三)因病殘而實際求診的比率(Use-disability rate),並以交叉列表方法觀察,此三指標與家長教育程度,家庭收入及家長有無健康保險間之間係,結果顯示臺北市居民有極高的未滿足醫療需要,這主要受家庭收入的影響,去除經濟上的障礙當能增加醫師求診的平均頻次並減低未滿足的醫療需要。 本研究使用之方法及結果可供為衛生計劃之工具評估居民對醫療服務能用之狀況。 |
英文摘要 | The almost unbelievably rapid advances in biomedical science during the twentieth century have not been accompanied by corresponding advances in health and medical care administration and planning. This resulted in a sideling gap between the actual delivery of health services and health services we should be able to deliver in view of present biomedical knowledge. People’s utilization of the existing health and medical services would be an indicator whether the services have met the needs or not. It is presumed that if the services could not meet the needs, there would be a gap between the existing services and the utilization of such services. To test this and related hypotheses, the study was designed to measure the accessibility of medical care in Taipei City. Three measures of access to medical care were employed in this study: a) Probability of Physician Visitation, b) Mean Physician Visits, and c) Use-Disability Rate. Totally 16 Lins (574 households) were sampled from 16 Districts of Taipei City at random cluste sampling. The head of family was directly interviewed by well trained interviewer. A structured questionnaire was used for interview. Cross tabulation of rates was used to report the relationships between three measures and family income, education of head of the family, and health insurance. The study suggests that the citizens of Taipei City do not enjoy high level of access to medical care and they also receive inadequate care relative to their medical need. Family income is the most important determinant of access to medical care. |
