| 中文摘要 | 背景:國外文獻顯示,重症加護病房(ICU)倖存者出院後會出現重症加護後 症候群(PICS)。然而,很少有研究探討台灣 ICU 倖存者的 PICS 問題和相關因素。 目的:本研究旨在探討 ICU 倖存者的重症加護後症候群及相關影響因素。 方法:採用橫斷面設計,研究樣本為台灣南部某醫學中心 126 位 ICU 存活者參 與本研究。測量工具包括重症加護後症候群量表、台灣簡明疲憊量表、睡眠品質 評量尺、個人基本資料和疾病相關變項。 結果:本研究結果顯示研究參與者平均年齡為平均年齡為 66.1 歲,其 PICS 得 分 與 年 齡(r = .31)、ICU 持 續 時 間(r = .32)、 再 入 ICU(r = .28)、 呼 吸 器 使 用 天數(r = .38)、疲勞(r = .87)之間存在顯著正相關(全部 p < .01);睡眠品質與 PICS 之間呈負相關(r = -.32,p < .01)。逐步迴歸顯示疲憊是 PICS 的重要預測因 子,總變異量為 79.1%。 結論:研究結果顯示 ICU 存活者經歷過 PICS;此外,疲憊是 PICS 的主要預測 因子。醫療照護者可以評估和監測 ICU 倖存者的疲憊程度,並教導患者及其家人 有關睡眠的重要性以及改善睡眠品質與疲憊程度以減少 ICU 倖存者的 PICS。 |
| 英文摘要 | Background: Previous studies demonstrated that intensive care unit (ICU) survivors suffered post-intensive care syndromes (PICS) after discharged from hospital. However, few studies have explored the issue and relates factors for ICU survivors in Taiwan. Aims: The purpose of this study was to explore the post-intensive care syndromes and related factors on ICU survivors. Methods: A cross-sectional design was used, and 126 participants who were survivors from the ICU were recruited from a medical center in southern Taiwan. The outcome measurements included a post-intensive care syndrome scale, the Brief Fatigue Inventory- Taiwan, sleep quality scale, personal characteristics, and disease-related variables. Results: The mean age of participants was 66.1 years. Significant positive correlations were found between PICS and factors such as age (r = .31), ICU length of stay (r = .32), ICU readmission (r = .28), and used ventilator days (r = .38) (all p < .01). Sleep quality was negatively correlated with PICS (r = -.32, p < .01). Stepwise regression indicated that fatigue was a major predictor of PICS, explaining 79.1% of the variance. Conclusion: The study results demonstrate that ICU survivors experience PICS; in addition, fatigue is the major predictor of PICS. Healthcare providers can assess and monitor the level of fatigue in ICU survivors, and educate patients and their families about the importance of sleep hygiene and strategies to improve sleep quality and fatigue level for decrease the PICS in ICU survivors. |
