| 中文摘要 | 目的:以品管圈手法分析門急診藥局調劑臨時撥補率高的原因,改善藥師因臨時撥補導致調劑中斷和取藥來回奔波之疲累,減少調劑錯誤之風險。方法:本品管圈由北部區域教學醫院跨部門組成,圈活動期間為2022年4月至2023年4月。收集門急診藥局調劑臨時撥補狀況、進行解析及真因驗證,擬定三大對策群組:(1)麻雀雖小、五臟俱全;(2)心智共享、守望相助;(3)支援前線、用進廢退。結果:對策實施前與對策實施後結果顯示,門急診藥局調劑臨時撥補率由1.86%下降到0.97%,門急診藥師調劑臨時撥補次數由969.86次/月下降到569.50次/月。藥師工作身心疲累度4與5分(重度疲累)比例由53.92%降低至20.26%。結論:運用品管圈優化庫存管理及調劑作業,成功減少門急診藥局調劑臨時撥補率及降低藥師身心疲累度。 |
| 英文摘要 | Objective: To analyze and improve the unscheduled dispensing replenishment rate in outpatient and emergency pharmacy. This aims to reduce pharmacists' fatigue caused by dispensing interruptions, decrease the risk of dispensing errors, and enhance dispensing efficiency and accuracy. Methods: This Quality Control Circle (QCC) was established by cross-departmental teams at a regional teaching hospital in northern Taiwan, with activities conducted from April 2022 to April 2023.Data on unscheduled dispensing replenishments were collected and analyzed. Root causes were identified, and three strategies were implemented: (1) increasing the number of stocked drug items, (2) regularly reviewing stocked items and adjusting inventory levels, and (3) ensuring sufficient medication supply and optimizing inventory management. Results: The outcomes observed before and after the implementation indicated that the unscheduled dispensing replenishment rate decreased from 1.86% to 0.97%. Pharmacists' trips to the inventory decreased from 969.86 times/month to 569.50 times/month. The percentage of pharmacists reporting severe burnout decreased from 53.92% to 20.26%. Conclusions: By applying the Quality Control Circle to optimize inventory management and dispensing operations, we successfully reduced the unscheduled replenishment rate and alleviated the pharmacist's physical and mental fatigue. |
