| 中文摘要 | 在現代醫療中,安全、精準、高效的輸液管理,是護理品質的重要保證。隨著醫療技術的迅速發展,輸液幫浦作為常見的醫療設備,直接關係到病人的用藥安全與醫療效率,不僅需要滿足臨床需求,還需要與智慧醫療系統相結合,以實現數據化與流程自動化。運用藥品防錯系統(dose error reduction system, DERS),和具備防止劑量錯誤藥典庫的智慧連線型輸液幫浦,實為護理師提供決策支援,成為醫療機構提升給藥安全的重要工具,本院經評估後,決定全體系導入智慧連線輸液幫浦系統,由護理部、藥劑部、醫學工程部、資訊室、總務室等跨部門組成專案團隊,將藥品醫囑系統、條碼給藥系統及連線型智慧輸液幫浦,整合至化療給藥過程,取代手動調整輸液幫浦參數所造成給藥錯誤,降低護理師耗時劑量計算和記錄,專案執行後,發放線上自擬問卷,共計回收187份,採用李克特量表(Likert scale)5分法,7個構面依分數高低為教育訓練(4.03)、臨床效能(3.96)、系統功能(3.93)、使用功能(3.8)、整體設計(3.4)、介面操作(3.15),平均值介於3.15至4.03分。連線使用率約為98.4%,成功率是99.8%,給藥流程時間縮短49.8%,大幅提升工作效率,讓護理師有更多的時間照顧病人,並確保病人正確安全地用藥,提升醫療品質。此制度的變革,翻轉給藥流程,落實以病人為中心的照護,是本院邁向卓越醫療的一大步! |
| 英文摘要 | In modern healthcare, safe, precise, and efficient infusion management is a key indicator of nursing quality. With the rapid advancement of medical technology, infusion pumps-widely used clinical devices-play a critical role in both medication safety and healthcare efficiency. These devices must not only meet clinical requirements but also integrate with smart medical systems to achieve digitalization and process automation. The implementation of smart, connected infusion pumps equipped with a Dose Error Reduction System (DERS) and a built-in drug library capable of preventing dosing errors offers valuable decision support for nurses and has become an essential tool for enhancing medication safety in healthcare institutions. After comprehensive evaluation, our hospital launched a full-scale adoption of the smart infusion pump system. A cross-functional project team was formed, involving the Nursing Department, Pharmacy Department, Biomedical Engineering, Information Technology, and General Affairs. The team integrated the electronic medication order system, barcode-assisted medication administration, and the smart infusion pump into the chemotherapy infusion process. This approach replaced manual infusion rate adjustments, reducing medication errors and minimizing the time nurses spent on dose calculation and documentation. Following the implementation, a self-designed online questionnaire was distributed, and 187 responses were collected. A 5-point Likert scale was used to evaluate seven dimensions. The scores, from highest to lowest, were: education and training (4.03), clinical effectiveness (3.96), system functionality (3.93), usability (3.80), overall design (3.40), and interface operation (3.15), with average scores ranging from 3.15 to 4.03. The system achieved a connection usage rate of 98.4% and a success rate of 99.8%. Medication administration time was reduced by 49.8%, significantly improving workflow efficiency and allowing nurses more time for direct patient care, while ensuring safe and accurate medication delivery. This transformation of the medication administration system represents a major step forward in our hospital's pursuit of excellence in patient-centered care. |
