| 中文摘要 | 本文描述一位38歲慢性思覺失調症病人,入院時發現因病程緣故,有明顯負性病徵所致生活作息不規律、多躺床、拒絕動態活動且因長期服用抗精神病藥導致口慾增加,有現存性體重過重之健康問題,故引發筆者採用多元管理策略進行體重管理。照護期間自2022年10月07日至2023年01月15日,於建立良好治療性人際關係後,藉會談、病歷回溯及整體性護理評估工具收集資料,歸納護理問題為:營養不均衡:多於身體所需要、知覺感受變異及無效性因應能力缺失等。本經驗透過多元的管理策略包含:引領跨醫療團隊照護方式探討長期服用抗精神病藥所致體重變重之風險,進而調整藥物治療方針,以及提供飲食及運動管理策略(含:個別性的日誌、了解飲食熱量及敦促多元的體能活動參與等),最終在多面向的減重管理策略,達成目標,希望透過此經驗可提供慢性精神科病房照護人員,照護相似病人之照護參酌。 |
| 英文摘要 | This paper presents the nursing experience of managing overweight in a 38-year-old patient with chronic schizophrenia. Upon admission, the patient exhibited pronounced negative symptoms attributable to the course of the illness, including irregular daily routines, prolonged bed rest, and refusal to engage in physical activities. Additionally, long-term use of antipsychotic medications had led to increased appetite, leading to a health problem of being overweight. These factors prompted the author to implement diversified management strategies for weight control. The care period began from October 7, 2022 to January 15, 2023. After establishing a therapeutic interpersonal relationship , data were collected through interviews, review of the patient's medical records, and the use of holistic nursing assessment instruments. The following nursing problems were identified: imbalanced nutrition-more than body requirements, sensory perception disturbance, and ineffective coping mechanisms. This nursing experience employed diversified management strategies, including leading a multidisciplinary team to assess the risks of weight gain associated with long-term antipsychotic use and subsequently adjusting the pharmacological treatment plan. In addition, dietary and physical activity management strategies were implemented, such as personalized dietary logs, education on caloric intake, and encouragement of diverse physical activities. Through these comprehensive weight management interventions, the target outcomes were achieved. This experience may serve as a reference for healthcare professionals in chronic psychiatric wards when caring for patients with similar conditions. |
