| 中文摘要 | 研究目的: 世界衛生組織在 2019 年 5 月的世界衛生大會批准國際疾病分類第十一版(ICD-11), 並於 2022 年 1 月 1 日生效,國際疾病分類第十一版(ICD-11)首度將中醫納入章節,ICD11 TM1(Traditional Medicine Module 1)以中國、日本、韓國的傳統醫學內容為主,中日韓 各國均投入國家資源進行相關應用研究。為助本國衛生資訊接軌國際並應用此章節,本 計畫目標為推廣國際疾病分類第 11 版(ICD-11)中醫藥應用,並提出相關策略、評估其 應用的可能性。希冀藉由此計畫執行,提升國內中醫藥學術、臨床及產業工作人員、學 術研究者、臨床醫(藥)師、中醫藥產業管理人員,健康醫療保險管理者等對於 ICD-11 的 認識及應用。 研究方法: 一、蒐集高度應用 ICD-11 TM1 國家相關研究,並召開專家座談會,綜合內容提出 未來臺灣推動 ICD-11 TM1 應用策略及實施步驟建議書。二、邀約中醫師填寫 ICD-11 TM1 模擬診斷問卷,統計結果分析後提出編撰中醫臨床診斷標準說明書之策略建議報 告書。三、辦理 2 場種子教師培訓工作坊,培育 ICD-11 TM1 種子教師,協助推行 ICD11 TM1 實施。四、邀請國際 ICD-11 TM1 編修專家學者辦理國際學者分享會,分享國 際推動經驗,及未來 WHO 推動傳統醫藥之策略建議。 結果與討論: 一、持續推動 ICD-11 TM1 中醫教育訓練。二、證型診斷前先建立診斷規則:事先 建立診斷規則,選擇直接與治療相關的證型診斷碼,或能夠反映費用高低,將病症與費 用做結合,未來也能夠提供給衛生福利部中央健康保險署參考。三、編修臺灣適用版的 ICD-11,累積實證基礎,進行國際交流。四、中藥適應症 ICD-11 編碼納入產品敘述, 快速對應證型,能夠有共通的國際語言。五、建立中西醫對照編碼。六、結合資訊科技 輔助 ICD-11 診斷。七、持續辦理種子教師培訓工作坊、建立 ICD-11 培訓基地:在未來 國內全面實施 ICD-11 之前,建議政府應持續支持 ICD-11 方面的相關研究,透過本計畫 的執行,發現國內中醫主治醫師師對於 ICD-11 TM1 並不熟悉,未來應持續辦理種子教 師培訓工作坊,持續擴大中醫藥界對 ICD-11 TM1 的認識。也於醫學中心建立 ICD-11 培 訓基地教導中醫師如何使用 ICD-11 診斷碼,並將建議提交相關單位。八、有系統的將 所有疾病進行模擬診斷。 |
| 英文摘要 | Aim: The World Health Organization (WHO) approved the Eleventh Edition of the International Classification of Diseases (ICD-11) at the World Health Assembly in May 2019, and it will take effect on January 1, 2022. ICD-11 is different from previous versions. Traditional medicine has been included in the chapter for the first time. ICD-11 TM1 (Traditional Medicine Module 1) is primarily based on traditional medicine content from China, Japan, and Korea. All these countries are investing resources in conducting ICD-11 TM-1 research. In order to align our country's health information with international standards and apply this chapter, this project aimed to explore the impact of ICD-11 on the development of traditional Chinese medicine in Taiwan and plans relevant strategies and countermeasures. Through the implementation of this project, we hope to enhance the understanding and application of ICD-11 among domestic professionals in the fields of Traditional Chinese Medicine, including academic, clinical, and industry practitioners, as well as researchers, clinical physicians (pharmacists), managers in the Traditional Chinese Medicine industry, and healthcare insurance administrators. Method: (1) Collect highly relevant research on the application of ICD-11 TM1 in the country and convene an expert symposium to consolidate content and propose a recommendation document for future strategies and implementation steps for promoting ICD-11 TM1 application in Taiwan. (2) Invite Traditional Chinese Medicine practitioners to complete a simulated diagnosis questionnaire for ICD-11 TM1, and after analyzing the results, present a strategic recommendation report for compiling a manual on clinical diagnostic standards for Traditional Chinese Medicine. (3) Conduct two workshops for seed teachers to train and cultivate ICD-11 TM1 seed teachers, assisting in the implementation of ICD-11 TM1. (4) Invite international experts and scholars in the editing of ICD-11 TM1 to organize an international scholar-sharing session, where experiences in promoting ICD-11 TM1 internationally will be shared, along with future strategic recommendations for WHO's promotion of traditional medicine. Results & Discussion: (1) Continue to promote ICD-11 TM1 traditional Chinese medicine education and training. (2) Establishing diagnostic rules is helpful for syndrome diagnosis. (3) Compilation and revision of ICD-11 TM1 suitable for Taiwan. (4) Apply ICD-11 TM-1 coding to indications of traditional Chinese medicine products. (5) Establish the ICD-11 TM-1 code that compares Chinese and Western medicine. (6) Combining information technology to assist ICD-11 TM-1 diagnosis. (7) Continue to conduct seed teacher training workshops. (8) Systematically carry out simulated diagnosis of all diseases year by year. |
